Keep reading to determine if this position is the one you have been waiting for! Employee in this position will evaluate abuse, neglect and exploitation of reports made on disabled adults. Provide essential case management until case closure with a plan to prevent further abuse, neglect or exploitation. Target population are adults at risk because of physical or mental incapacity; collaborate with health care (physical/mental) professionals, agency attorney, law enforcement, adult care facilities, Medicaid staff and community to obtain suitable care and services. Arranges for the provision of and assisting disabled adults and/or families in utilizing appropriate services and community resources. Social Worker in this position provides micro treatment in directing individual change necessary to prevent or remedy abuse, neglect, or exploitation. The treatment includes counseling, individual advocacy, crisis intervention and focusing on services which will meet the disabled adult needs and have the least possible effect on his/her chosen life style and sense of independence. Participates in internal and state reviews to ensure quality assurance in the Adult Protective Service program. Case management responsibilities may sometimes include transporting customers in the employees personal vehicle when called upon. Employee must demonstrate intermediate technology skills, attention to detail, be able to keep case management documentation accurate and current in a paperless system. Position must possess the ability to interpret and apply program policies for program compliance as part of their case auditing responsibility. Employee must possess valid drivers license and dependable transportation. Master's degree in social work from an accredited school of social work; Bachelor's degree in social work from an accredited school of social work and one year directly related experience; Master's degree in a human services field and one year of directly related experience; Bachelor's degree in a human services field from an accredited college or university and two years directly related experience; Bachelor's degree from an accredited college or university and three years of directly related experience PREFERENCES: Prefer applicants with BSW or MSW Degrees from an accredited college or university; or social work experience in a Department of Social Services and Human Services Degree; or social work experience in a human services setting and a Human Service Degree; or a Human Services Related Masters Level Degree.
