Job Description Purpose of Position: Under supervision of the Social Work Supervisor IV or his/her designee, is responsible for the Social Work Services of the Mobile Crisis Unit (MCU). Acts as a member of an interdisciplinary team to conduct: crisis assessments, de-escalate situations and make appropriate connections to care and provides advisory service relating to community issues and resources. Duties and Responsibilities:
- Provides clinical assessments for the Mobile Crisis Unit.
- Ensures adequate coverage and continuity of patient care at all times.
- Carry a caseload and adheres to Department's documentation policies and procedures.
- Provides information about and acts as linkage to available community resources.
- Ensures that staff documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies.
- Participates in the Department's cross training program
- Conducts and/or attends individual, unit, departmental, intra/inter hospital, and community meetings as mandated by the administration and or supervisor.
- Reviews Medical Records for MCU and other areas as indicated by the Social Work Department
- Home visits (Visits patients' homes as needed/indicated by the mobile crisis team).
- Ensures compliance with Child Protection policies, procedures and laws.
- Participates in the Hospital/Departmental emergency response to internal and external disasters.
- Performs other duties assigned by supervisor as needed.
Minimum Qualifications For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
