Job Description Examples of Typical Tasks: Provides telephone and in-person triages for families requesting care to determine appropriateness for outpatient services and urgency for appointment scheduling Conducts initial assessment/mental status examinations as part of the screening process Consults Psychiatrist if mental status reveals patient at risk for dangerous behavior with regard to self or others Conducts a full intake mental health assessment including review of medical and school records, developmental, educational, and psychiatric history, behavioral observations, and interviews with collateral caregivers Establishes a working DSM V diagnosis Writes progress notes and makes all appropriate chart entries regarding contacts for each assigned patient Develops and documents treatment plans with parent/caregiver and patient with quarterly re-asses Provides individual/group or family treatment with appropriate collateral involvement based on theoretical treatment model (carries active caseload of at least 30, each case involves visits with patient and caregivers) Consults with other staff members as needed Coordinates care with CAPS Psychiatrist to assure integrated treatment services Willingness to work and provide direct patient care virtual services on-site and/or remotely. Minimum Qualifications 1. Valid license as an LCSW issued by the NYSED and one (1) year of supervisory experience utilizing casework, group work and/or community organization methodologies.
