Hours: Per Diem
Shift: Day/Evening shift with weekend, at least one holiday per year, and on-call rotation
Location: St. Luke's Hospital - New Bedford, MA; Position will follow a hybrid schedule

ResponsibilitiesUnder the direction of the Executive Director of Integrated Care Management performs a wide variety of duties in Southcoast Physician Practices providing behavioral health and complex care management services to Southcoast Health and Southcoast Health Network PCP patients across the system. Duties include conducting behavioral health assessments and providing behavioral health treatment including individual and family psychotherapy utilizing brief treatment modalities. Serves as a consultant and resource to practice staff in the management and treatment of behavioral health disorders. Collaborates and coordinates with complex/chronic care management and longitudinal community support servicesQualificationsMust be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
