Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations.The Social Worker at the Seaford Boys Group Home must be a licensed social worker (LMSW or LCSW), providing informal clinical support and case management for up to 7 youth residing in the home. Maintaining a focus on the overall mission of the agency, and the Residential Department, the licensed social worker/Program Manager is supports and empowers our youth and their families to build resiliency and become self-sufficient to achieve their fullest potential. Work includes individual meetings, family and permanency team meetings, home visits, collaboration with community partners, and crisis intervention.-NEW HIRE BONUS: $500 AND $750 IF YOU ARE BI-LINGUAL in English/Spanish!!To be a successful Social Worker you must be ready, able, and willing to do the following always:
- Function within the policies and procedures as outlined in the program practice guide and employee manual.
- Provide overall management of assigned site.
- Complete necessary reports and case documentation.
- Liaise with DSS, schools and other child and youth collateral contacts.
- Provide crisis intervention and counseling as needed.
- Provide supervisees with the knowledge, skills and tools needed to succeed
- Adhere to state regulations and applicable code of conduct.
- Preparing psychosocial assessments.
- Collaborate and coordinate with the aftercare caseworker on discharge planning and aftercare services
- Conducting home visits.
- Support other projects and residential program locations, as needed.
- Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations
How you will typically spend your time in this job:
- 50% General casework responsibilities, documentation, reports
- 20% Supporting youth
- 20% Supporting staff
- 10% Staff meetings/supervision/training
Minimum qualifications for this job are:
- LMSW / LCSW required
- 2 years' post-master's experience preferred
- Experience with child serving systems such as DSS, probation, Foster Care Systems
- Computer proficiency in Microsoft Office Suite, including MS Word and MS Excel
- A valid NYS driver's license in good standing
- Ability to provide 24/7 on call availability
- Additional physical requirements: Operate a motor vehicle carrying up to ten (10) children and youth, lift/move up to fifty (50) pounds, escort children and youth into the community using stairs when necessary, restrain a child or youth during a crisis, read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements
