A health services organization in New York City is currently seeking a new Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW) to join their Mobile Integration Team (MIT).Responsibilities:The Social Worker (LMSW / LCSW) will be responsible for:
- Providing Support and Clinical services to the assigned clients within the guidelines of Social Work professional Values and Ethics, NYS OMH and MPC Dept. of Social Work
- Providing outreach and engagement to motivate the clients to be involved in treatment
- Functioning as resource to provide linkages to community-based resources; completes any referrals necessary for services
- Providing crisis interventions and ego support services
- Providing individual or group supportive therapy
- Maintaining accurate and timely documentation, including 725s, 116s, Individual Service Plan, Service contacts and MHARS documentation
- Attending daily meetings, discuss pertinent treatment needs/options, progress with the MTI Team about the assigned clients
Qualifications:
- 1+ year of relevant work experience
- Active NYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)
