Work Shifts 9:00 A.M - 5:00 P.M Duties & Responsibilities PURPOSE OF POSITION: Under supervision, provides social work services and the coordination of continuing care planning, utilizing a variety of techniques such as individual, family, group work, and community networking to patients serviced at this facility. AREAS OF RESPONSIBILITIES: Weekend and Holiday Medicine Inpatient Coverage. SUMMARY OF DUTIES AND RESPONSIBILITIES:
- Screens the medical records of outpatient and admitted patients in order to identify those whom will require social work intervention and discharge planning services.
- Interviews patient and where appropriate family member(s)/significant other(s) to assess their ability to function in the community.
- Completes Social Work Comprehensive Assessment for high/moderate-risk patients.
- Devises/implements a social work treatment/discharge plan which involves the patient, family/significant other and the interdisciplinary team.
- Documents Social Work Assessment using EPIC
- Documents all Social Work interventions in the patient's electronic medical record (EPIC) in accordance with departmental and hospital policies.
- Confer and consults with clinical staff in the development of the discharge plan.
- Establishes priorities and accomplish multiple tasks.
Minimum Qualifications For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
