Together, we improve the health of patients and their families, making our communities - and the world - strongerThe Pediatric Social Worker is to provide discharge planning for patients admitted to inpatient pediatrics and the pediatric intensive care unit (PICU). Duties include:
- Conduct psychosocial assessment with child and family
- Secure community services that will be needed post discharge such as homecare, medical equipment, outpatient mental health services, or inpatient rehabilitation in a skilled nursing facility.
- Help parents cope by providing education, counseling, and other helpful resources.
- Identify child abuse and neglect and make reports to the State Central Registry
- Arrange transportation when needed.
- Work with pediatric psychiatry to develop safe discharge plan
- Obtain psychiatric bed for patients in need of inpatient psychiatric treatment. This includes getting insurance auth, coordinating with the family, finalizing transfer with NYP Access Center.
- Participate in interdisciplinary rounds
Shift: M-F, 9-5, with one weekend per month; 1199 position
Preferred Criteria- Experience as an inpatient social worker
- Experience working with pediatric patients
- Experience using Epic and Allscripts
Required Criteria- Masters Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW)
