Job Description SummaryProvides comprehensive and continuing patient care through the provision of social work care coordination to patients and their families. Assesses data gathered in terms of the medical plan of treatment, available social service programs and plans and implements discharge arrangements.Educates patients and families in understanding, accepting, and following medical recommendations.Initiates screening of discharge planning needs on high-risk patients. Coordinates discharge planning activities with input from the appropriate inter-disciplinary team. Stays abreast of available community resources and utilizes resources to help patients in smoothly moving to the next appropriate level of care. Interprets the nature of the illness and prescribe course of treatment to patient and family. Communicates family issues to other staff members one to one and in patient care conferences.Coordinates with the inter-disciplinary team members and community resource personnel.Adheres to the department policy for documentation and provides monthly statistics as required.Provides crisis and situation-oriented counseling to patients/families.Performs other miscellaneous and related duties as required.Physical Requirements:Physical requirements are primarily related to the essential functions of any job. In order to perform the essential functions of this position, you must be able to stand, walk sit, use hands, stoop, talk, and hear. Must be able to lift or exert energy up to 10 pounds 100% of the time. Must have close vision (clear vision at 20 inches or less), and distance vision (clear vision at 20 feet or more).Mental Requirements:Must possess the ability to read and interpret business or safety manuals, clinical or technical procedures or regulations. Ability to write reports, correspondence and/or manuals. Must have the ability to effectively present information and respond to questions from groups, patients, families, other employees, and medical staff. Must have the ability to perform more complex math functions to include calculating percentages, averages, volume, ratio and interpreting graphs-using numbers, money and units of measurement. Must have the ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to deal with concrete and abstract concepts and interpret verbal, nonverbal and written instructions.Age Groups:This position primarily interacts with children, adolescents, adults and aging adults and provides direct patient care. All staff members must be able to demonstrate the knowledge and skill necessary to meet the physical, psycho-social, educational, safety, and related needs of the persons in the assigned work area.Additional Job DescriptionMinimum Qualifications:A Degree in Social Work. LMSW, LISW, or LBSW license with the State of South Carolina required within 6 months from date of hire.
