Duties and Responsibilities:
- Provides services designed to ensure that the medical, social and emotional needs of each registrant are provided for.
- Works together with the team (psychological, medical, recreation, nursing, physical therapy, and occupational therapy, dietary) to identify potential difficulties for the registrant, provides crisis intervention and is available to registrant and families for counseling, or other psychosocial services, as indicated.
- Assists new registrant and their families in the initial adjustment phase, as well as arranges admission interviews to gather psychosocial information necessary to the completion of the initial assessment
- Facilitates care plan meetings with registrant and team.
- In conjunction with the interdisciplinary team, completes the registrant assessments, and develops an individualized plan of care for each resident.
- Works with registrant and families, where indicated, on planning for services in the community, or to another environment, as dictated by the needs of the individual registrant. Assists the resident and/or family in accessing community and government services.
Requirements:
- Bachelors Degree in Social Work or Masters Degree in Social Work and related social sciences
- Experience in a Skilled Nursing Facility (SNF) setting or Adult Day Health Care (highly preferred)
- Applicants should be proficient and competent with excel and healthcare operating systems
Experience:
- Social Work: 2 years (Preferred)
Education:
Bachelors of Social Work, Masters of Social Work (Preferred)
Location:
- Brooklyn, NY 11236 (Preferred)
Schedule:
- Monday to Friday 9am 5 pm
