People who are struggling with their mental health need compassionate Healthcare Workers now more than ever!Job Summary: As an independent practitioner, responsible for providing clinical services/counseling, screening, and case management services to patients and their families, within the Virtual Adolescent PHP/IOP, in order to promote the optimal individual, interpersonal and social functioning of the patient in their community.QUALIFICATIONS:1. Master's degree in Clinical Social Work from an accredited graduate school of Social Work or Master's degree in counseling or other behavioral health discipline.2. Social Worker II must have a valid Vermont LICSW, LMFT, LCMHC or LADC3. Possession of a valid driver's license and access to a private vehicle for day-to-day job performance.4. Some working knowledge of computer skillsESSENTIAL DUTIES AND RESPONSIBILITIES:Core CompetenciesEngagement, Screening, and AssessmentAssesses patients comprehensively, according to hospital and departmental standards. Assessments include a comprehensive understanding of the patient in context, including diagnosis, formulation, risk assessment, level of care assessment, and referral needs identified.Assesses family structure, needs, and functioning in context of patient's presenting problemsAssesses and plans for the patient's aftercare needs, in all levels of careAssesses family's needs with regard to discharge plan, when family is engaged with care of patientsHas ability to conceptualize patients and systems in their social context, taking into account systems stressors as well as intra- and interpersonal problems and strengthsDemonstrates understanding of addiction and recovery, including as it relates to screening, assessment, engagement, clinical interventions and approaches, and ASAM criteria for referrals and placementSynthesizes and conveys relevant information, include clinical and case formulations, orally and in writing.Formulates understandings of case material, using all data available as well as understanding of interplay between stressors and vulnerabilities and presenting problems.Understands best practices for assessment and treatment of clients with diverse backgrounds and cultural experiences, including intersectionality and the impacts of systemic oppression and marginalizationAssesses and diagnoses patients, using DSM 5 criteria, consulting with team members as necessaryDemonstrates the ability to form a therapeutic relationship with the patient and families.Contacts the patient's providers and referral sources to obtain collateral information for assessment, and to coordinate treatmentTreatmentDemonstrates the ability to provide direct clinical care to patients in order to promote change according to the patient's treatment plan.Individual Psychotherapy: Meets with patient in a therapeutic relationship to address clinical and psychosocial issues relevant to the patient's diagnosis and presenting problems, and promotes change according to treatment goals.Supportive therapy: Meets with patient as necessary to provide support and resolve specific task oriented problems that may have an impact on treatment.Group therapy: Meets with patients in psychoeducational or psychotherapeutic group sessions to promote, through the group process, individual growth according to treatment goals.Demonstrates the ability to integrate theoretical knowledge of clinical practice, group dynamics and psychopathology effectively and ethically in treatment.Attends and participates in treatment planning sessions of assigned patients on a regular basis. Reports consistently to the treatment team with regard to assessments and the content and focus of therapy.Documents all interactions with patient(s) in progress notes in the electronic medical record. Documentation will follow Brattleboro Retreat and departmental policies.Maintains contact with referral sources and involved community agencies according to the aftercare needs of the patient, using professional judgement and according to the patient's treatment plan, as coordinated with other treatment team members.DayFTE 1/40 Hours
