Responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings.Minimum Requirements:- Minimum of a Bachelor's degree in a health related field including but not limited to sociology, counseling, human services, community wellness and psychology.- One year of experience in a health care setting working directly with individuals.- Must demonstrate knowledge of the issues and developmental stages associated with geriatric residents. - Must be able to assess data related to age-specific needs and provide care as described in the department's policies and procedures.- A minimum of one year's social work experience in a health care setting.
