As a Social Worker, you will play a crucial role in providing recovery-oriented services to individuals with serious mental illness, predominantly Hispanic, residing in the Washington Heights neighborhood of Upper Manhattan. Responsibilities:
- Carry a caseload of patients with serious mental illness.
- Provide case management and supportive therapies to clients.
- Formulate multidisciplinary treatment plans in collaboration with the clinical team.
- Conduct psycho-social assessments and maintain accurate progress notes.
- Collaborate effectively with psychiatrists, nurses, peer specialists, occupational therapists, and other social workers.
Requirements:
- Active licensure as a Social Worker.
- Proficiency in Spanish preferred.
- Experience working with individuals with serious mental illness preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively within a multidisciplinary team.
