Job Summary:
As a Licensed Practical Nurse (LPN), you will be responsible for providing professional nursing care to patients in various healthcare settings. You will assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. You will also administer nursing care to ill, injured, convalescent, or disabled patients, and may advise patients on health maintenance and disease prevention or provide case management.
Job Responsibilities:
- Patient Care:
- Assess patients' conditions, noting any changes and responding accordingly.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and record patient vital signs and medical information.
- Assist with diagnostic tests and procedures.
- Care Planning:
- Develop individualized care plans based on patient assessments and collaborate with healthcare team members.
- Modify care plans as needed to reflect changes in patient status.
- Patient Education:
- Educate patients and their families on disease prevention, treatment options, and post-hospital care.
- Provide guidance and support to patients in managing their health conditions.
- Documentation:
- Maintain accurate and detailed medical records of nursing observations and care provided.
- Ensure compliance with documentation and reporting guidelines.
- Communication:
- Communicate effectively with patients, families, and healthcare team members.
- Collaborate with physicians and other healthcare professionals to plan patient care.
- Quality Improvement:
- Participate in quality improvement initiatives to enhance patient care outcomes.
- Follow established nursing standards, procedures, and protocols.
- Emergency Response:
- Respond effectively to medical emergencies, including administering CPR and providing emergency treatments.
- Patient Advocacy:
- Advocate for patients' rights and ensure their voice is heard in healthcare decisions.
- Address patient concerns and provide emotional support as needed.
Qualifications:
- Graduate of an accredited nursing program with a current NY Licensed Practical Nurse license.
- BLS/ CPR Certification.
- Fingerprinting.
- Up-to-date medical documentation(i.e. Physical, Titers and QuantiFERON)
- Excellent knowledge of nursing care methods and procedures.
- Strong critical thinking and problem-solving skills.
- Compassionate and caring demeanor.
- Excellent communication and interpersonal skills.
