- The Home Health Specialty Program Access Coordinator (SPAC) has operational and clinical oversight over transitional care for patients enrolled in Penn Medicine at Home (PMAH) specialty programs.Specialty programs at PMAH involve clinical protocols and care pathways meant to standardize care delivery in the home, meeting the needs of complex patient populations who are not always suited for the traditional home health model of care. PMAH specialty programs may require any combination of 1) same-day admission, 2) remote patient monitoring, 3) after-hours triage, 4) urgent care, 5) advanced rehabilitation services, and 6) advanced social services.The SPAC uses critical thinking skills along with advanced operational and clinical knowledge to ensure patients have appropriate physician orders, the appropriate complement of in-home and remote clinical services scheduled, clinically relevant supplies and equipment in the home, and are appropriately triaged and prioritized so as to create a seamless transition from hospital, ED, or community.The SPAC collaborates across teams and disciplines to plan, implement, and evaluate the effectiveness of clinical care information to promote superior patient outcomes. The SPAC supports the PMAH teams with prompt referral management and entry, communication, timeliness of scheduling, safety, and appropriate utilization of resources. The SPAC works closely with hospital-based case management, clinical and operational leaders, pharmacists, schedulers, field and office-based clinicians, and others to ensure these urgent referrals are processed, scheduled, and visits occur.
Responsibilities:
- Manages workflows and processes to ensure efficient, timely, high-quality referral management and data entry.
- Exhibits in-depth clinical and operational knowledge across PMAH agencies, including hospice, home health care, palliative care, and infusion therapy
- Provides clinical oversight to specialty programs, promoting critical thinking and optimal clinical decision-making to enhance patient safety, quality, and clinical outcomes.
- Closely collaborates with hospital, ED, and community-based providers and case management.
- Advocates for patients and ensures safe transitions in care by obtaining appropriate physician orders and procurement of DME and other supplies.Advises referral sources and provides access to the full complement of services and specialty programs throughout PMAH.
- Works closely with PMAH Leadership in developing and implementing clinical protocols, clinical programs, and IS/EMR enhancements to support and enhance the efficiency and effectiveness of specialty programs.
- Ability to informally lead others and work in a matrix organizationMeasures and monitors schedules to meet patient needs and manage clinical staff and provider expectations.
- Communicates effectively with patients, families, caregivers, and clinical staff while providing relevant and appropriate scheduling and clinical information.
- Demonstrates compassion and knowledge in all interactions with patients, family, and caregivers as patients transition onto PMAH specialty programs.
- Demonstrates working knowledge of patient safety principles, quality improvement theory, and regulatory policies
- Participates in relevant chart audits, compliance reviews, and RCAs/ACAs
- Demonstrates an ability to communicate effectively with staff, patients, and families understands organizational operations, including service recovery and customer satisfaction.
Credentials:
- Registered Nurse - PA or NJ (Required)
Education or Equivalent Experience:
- Bachelor of Science Nursing (Required)
- 5+ years Home Care, Hospice, Infusion direct care with leadership experience such as supervisor, lead, charge nurse, unit council chair
