Job DescriptionJob Description Provides comprehensive speech-language pathology services to patients in outpatient, hospital and skilled nursing facility setting and participates in the total plan of care as part of the health care team or director by departmental supervisor.Job Responsibility1. Outpatient: •Scheduling and maintaining daily schedule. •Physician initial order or referral. •Diagnosis for therapy services including date of onset; initial evaluations and all re-evaluation in accordance to Medicare and Insurance guidelines. •Plan of treatments with MD signature pertaining to claim period. •Progress notes and attendance records pertaining to the claim period to support total minutes of actual treatments and number of minutes for each modality •Clinical progress notes. •Perform videlfluroscopic swallowing studies. 2. Hospital: •Patient education sheet. •Multidisciplinary care plan. •Education for nursing staff and PCTs. •Updated forms and policy. •Perform standardized patient testing on each patient when appropriates. •Perform videofluoroscopic swallowing studies. 3. Skilled Nursing Facility: •Daily MDS. •Progress notes minimum one per patient, care plan and CAAs. •Daily screenings, SLP patient schedule, perform standardized evaluations, discharge summary. •Education for staff Nursing and PCTs and families. •Participation in SNF team meetings and dietary meetings. •Perform videofluoroscoic swallowing studies. • 4. Performs related duties, as required.*ADA Essential FunctionsJob Qualification•Master's Degree in Speech Pathology, required. •Currently licensed in Speech Pathology in the State of New York. •Certificate of Clinical Competency from the American Speech-Language-Hearing Association, required. •Eligibility for certification in Speech Pathology by American Speech and Hearing Association.*Weekend coverage.
