Success In This Role Looks Like:
- Overseeing services and support coordination for multiple sponsored homes in the community.
- Providing effective supervision and empowerment to direct care staff, fostering a team-oriented service delivery model.
- Ensuring the development and implementation of Individual Support Plans (ISPs) for residents, focusing on their holistic growth.
- Acting as a liaison between families, employers, and community resources to facilitate residents' success and integration.
- Maintaining compliance with licensing, accreditation standards, and agency policies in all residential homes.
- Supporting the social, spiritual, and moral development of residents, ensuring a person-centered approach to care.
- Ensuring host families in sponsored homes are up-to-date with required training and certifications.
- Managing the program's budget and providing timely reporting to maintain operational excellence.

What You Bring To The Team:
- Strong communication skills to effectively interact with adults with intellectual disabilities, their families, and other service providers.
- Experience in developing and implementing Individual Support Plans, with a focus on person-centered care.
- In-depth knowledge of residential programs for adults with developmental disabilities and support coordination services.
- Ability to work independently and manage multiple tasks while maintaining attention to detail and compliance.
- A commitment to empowering and supporting team members, ensuring the highest standards of care and service delivery.
- Bachelor's Degree in Social Work, Psychology or related field, or equivalent experience.
- Some experience working in the field of developmental disabilities.
- Meets Federal requirements to be a Qualified Developmental Disability Professional (QDDP).
