The individual must demonstrate knowledge of the principals of growth and development, and possess the ability to assess and interpret data to identify age specific needs. Participates in Natural Work Teams and/or Quality Action Teams. The purpose is to provide quality patient care by fostering therapeutic relationships, assisting in active treatment and supporting the hospital in the pursuit of the LSH Strategic Plan.Employees in this classification must be able to lift, transport, move or physically assist people receiving services either individually or with assistance. Employees must be able to lift 40 pounds repeatedly to accomplish these tasks and may occasionally be required to lift more than 40 pounds. In some instances, mechanical equipment may be provided to assist in lifting and moving people receiving services. Employees must have the physical strength necessary frequently push, pull, twist, bend squat, walk or stand for extended periods of time. Transporting people receiving services is also required, so walking, standing and sitting are essential to the job. Employees in this classification must also respond to aggressive resident behavior using specific techniques, which may at times include the use of physical restraint. Employees must be able to observe, hear, communicate and respond to people in distress. Employees in this classification may occasionally be required to work overtime, with shifts lasting up to a maximum of 16 hours per day. An employee in this classification escorts residents on the SPTP program to and from classes. Employee must be able to with stand frequent climbing of stair and stating longer than 10 minutes in one position.Job Responsibilities may include but are not limited to the following:Provides direct patient/resident care and fosters therapeutic relationships.* Learns patient/resident goals/objectives and documentation process.* Observes patient/resident behaviors to detect, report, and document significant behaviors and signs of stress* Gathers and documents data specific to behavioral modification and treatment plan goals.* Provides clear and direct feedback to patients/residents about issues and directs them to appropriate staff for consultation/resolution.* Completes all required documentation, (i.e., Incident reports, shift notes, daily charting, VS, etc.) in accordance with hospital and program policies, in a timely manner.* Interacts frequently with patients/residents, by encouraging participation in unit programming.* Encourages patients/residents to develop an understanding of their treatment goals and apply identified treatment strategies.* Promotes patient/resident independence, productivity and choice making.* Intervenes to prevent injury to patients/residents or others per LSH de-escalation techniques, using CPI approved procedures.* Accounts for patient's/residents presence per location monitor policy.* Takes vital signs as assigned, documents and reports abnormal findings to higher skilled nursing staff.* Trains, monitors and assists patients/residents with personal care and grooming, and activities of daily living.* Identifies teaching opportunities, providing timely instruction in conflict resolution, problem solving, stress management and social skills.* Collects laboratory samples for tests as directed by medical staff.* Performs blood glucose monitoring when trained and proficient.Maintains safe, secure, and therapeutic environment* Maintains safety for patients/residents on living units, at meals, in classrooms, at work sites, during activities, at program* Ensures that unit rules are maintained in a fair and consistent manner.Provides escorting and/or transporting of patients/residents to and from assigned activities/sessions, yard, callouts, and appointments off and on campus as directed.* Supervises all movement to ensure patient/resident safety.* Follows all policies and ensures supervision of resident/patient during all transports and/or escorts.* Considers patient/resident well-being, physical limitations and security risk when escorting.* Informs/prepares patients/resident for escort* Escorts patients/residents to locations and appointments on campus and off grounds as assigned.* Provides supervision of LSH patients/residents hospitalized in other facilities.* Provides transport for patient/resident via state vehicle in a safe & secure manner and follows agencies and program policies.* Provides clear and appropriate feedback to unit nurse or medical staff regarding verbal/written reports from on/off ground consultants.* Provides supervision of LSH patient/resident hospitalized in other facilities as assigned/trained* Runs errands to other areas on campusTeam Work / Process Improvement* Provides appropriate feedback concerning patient/resident care issues to members of treatment team.* Maintains daily e-mail access to receive communication from nursing department and others, providing expected follow-up.* Complies with LSH and Program policies, procedures and nursing standards.* Attends and provides input to unit treatment team meetings and staff meetings as assigned.* Participates in staff development and meets individual learning needs, to include required computer based training.* Mentors other staff by providing individual supports and training.* Participates and cooperates in Natural Work Teams and/or Quality Action Team as assigned.* Demonstrates willingness to learn new concepts and a positive attitude by working cooperatively in a team process.* Demonstrates caring and respectful communication to all internal and external customers.* Demonstrates acceptable work habits that include coming to work on time and as scheduled.* Adheres to the behavioral and ethical standards of patient/resident care.* Performs all other job-related duties willingly as assigned by supervisor to accomplish the Strategic Plan of
