SUMMARY: The Sr. Prevention Services Coordinator [PSC] is a vital member of the HIV Integrated Care Team and will facilitate 1) all internal and external screening engagement activities and events 2) PrEP services, and 3) education dissemination. The PPC assists the Chief Medical Officer and the HIV Manager in implementing and maintaining a successful HIV[Prevention] Program. The PSC works with grant and program leadership to ensure that grant deliverables are met, program initiatives are successful, and all activities meet extensive state and federal funding requirements to ensure continued funding and program expansion. ESSENTIAL DUTIES AND RESPONSIBILITIES: Commit to and apply the Mission and Values of ARMSAssist in the development of new screening workflows, policies and procedures, protocols, standard operating procedures, staff training materials and other relevant documents that guide care delivered within the HIV prevention programAssist HIV Manager to oversee and maintained the HIV Program workplan Develop and implement a community outreach/in-reach education/engagement plan for HIV, STD, hypertension and diabetes screening/ prevention servicesDevelop and implement a care coordination and linkage to care process to ensure patients receive needed care In partnership with the outreach and mobile health unit teams, plan and coordinate local events for National HIV Testing Day (June) and World AIDS Day (December 1st) Increase PrEP awareness and knowledge and assist community members initiate PrEPWork to build the trust and confidence of patients/persons to overcome fears or anxiety related to their health condition(s) or with navigating the health systemUtilize motivational interviewing techniques to assist patients in understanding their vulnerabilities for acquiring HIV and the use of PrEP as a practical HIV prevention toolProvide patient and insurance navigation services and PrEP services (e.g. HIV screening, rapid access appointments, confirmatory blood testing) to patients/persons interested in starting PrEPWork closely with ARMS providers, pharmacists and patient assistance programs to ensure patient prescriptions are filled in a timely mannerWork with patients initiating PrEP to assess needs/barriers, assist in the development of care plans and coordinate PrEP-related services through advocacy, support, and empowermentProvide follow-up services with patients to ensure adherence to the prescribed therapy, and remain engaged in PrEP care (e.g., scheduling quarterly medical appointments)Meet regularly with the ARMS HIV interdisciplinary team to discuss successes, challenges, and innovative approaches to address patient's needs and to improve PrEP servicesPerform required entry of patient level data and documentation of services, including HIV/STD results and referrals to enabling services, in the electronic health record/data systemAssist in the preparation of data and grant reports and power point presentations as neededAssist in writing required internal and external program reports including QI reportsDevelop announcements and marketing materials for specific programs in collaboration with program staff and ARMS Director of Outreach, Marketing, and Public AffairsAssist HIV Manager oversee community health worker staff including the facilitation and provision of training and ongoing professional developmentAttendance at required ARMS meetings and external meetings with other local providers. Participate in annual trainings and updates regarding HIV medical services and HIV prevention services Perform other duties as assignedEDUCATION AND EXPERIENCE: Bachelor's degree in social work, mental health or counseling required Master's degree preferred Two years of experience in facilitating HIV prevention and PrEP Demonstrated commitment and experience working with racial minorities; LGBTQ+ adults and youth; and economically diverse populations 3 years of program coordination experience; community/public health in ambulatory care; case management, navigation, education or human services 3 years of motivational interviewing experience Experience working with patients with other chronic and behavioral health needs Specialized training or experience in HIV prevention interventions, PREP/PEP, and STIs, a plus ALABAMA REGIONAL MEDICAL SERVICES is an EEO employer - M/F/Vets/Disabled
