Overview:

Conducts investigations of incidents and completes investigation reports. Acts as a mentor to other investigators, providing training and constructive support for ongoing investigations. Ensures quality of care of persons receiving services from the Agency by ensuring compliance with agency policies and procedures and OPWDD regulations.

Responsibilities:

Primary duties and responsibilities include but not limted to:

  • Conducts investigations of incidents and allegations of abuse. Analyzes information obtained during investigation and completes investigation reports; identifies systemic issues and provides recommendations for corrective measures
  • Manages files, and ensures investigative materials are uploaded to oversight agencies within regulatory timeframes
  • Provides general and specific training for Quality Assurance Coordinators and agency staff, as needed, regarding incident management and investigations
  • Works constructively with and mentors Quality Assurance Coordinators/Per Diem Quality Assurance Coordinators to develop investigation plans and provides feedback/support
  • Reviews investigation reports completed by Quality Assurance Coordinators/Per Diem Quality Assurance Coordinators to ensure thoroughness; provides suggestions for revisions and follows up to ensure completion
  • Contributes to the development of policies & procedures related to incident management
  • Provides support to the agencys Quality Improvement plan including participation in agency and outside entity survey activity as needed
  • Chairs minor notable incident review committee and provides oversight and tracking of minor notable incidents to ensure timely submission / completion
  • Additional tasks/duties as assigned by the Director of Quality Assurance
Qualifications:
  • BA or BS in human services field; 5 years experience supporting people with intellectual and other developmental disabilities.
  • Applicant must be a certified investigator with 1 year experience conducting investigations in an OPWDD certified setting.
  • Excellent organizational skills, interpersonal skills and prior experience training preferred.
  • Computer proficiency in Word and Access preferred.
  • Qualified NYS Driver's License required.
