Description I) Job PurposeThis role as the Sr. Service Coordinator monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory. Performs service management function for all residents needing assistance. Provides limited case management (i.e., evaluation of social, psychological and physical needs and the development of a service plan) for resident when such services are not being provided by the general service community. Educates residents on service availability, application procedures, residents rights, etc. for both individually and as a group. Reports all suspected abuse situations to the appropriate agency. Sets up volunteer support programs through MHMR organizations any other community aspects.II) Essential FunctionsA) Provide Service Coordination to a specific caseload (minimum of 18) and to provide support through example to new Service Coordinators (SCs) or SCs that are identified as needing additional skills. B) Demonstrate the understanding planning, program availabilities, goal setting, development the PDP, monitor services, and other aspects for all programs (i.e., Person Centered, Medicaid, Direct Choice Selection, Setting Rules, TAC and other appropriate handbooks, and others).C) Adhere to all rules and regulations for state, federal, MHMR and all other aspects that relate to position.D) Continue education (i.e., SC Assessment annually, TWC referrals, guardianship, CFC assessment as assigned) and other aspects of the profession. E) Crisis situations follow procedures and approve best practices (i.e., understanding differences between provider responsibilities and Authority's).F) Continue communication with all appropriate personal regarding (i.e., peer, staff, provider, authorities, Program Managers, and other).III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and AbilitiesA) MHMR policies and operating procedures. B) HHCS rules and regulations related to HCS/TxHmL and Billing Guidelines. C) Have experience and expertise in HCS as a SC.D) Ability to work independently as a team in both routine and crisis situations.E) Ability to communicate the role and tasks of a SC. F) Demonstrates good teaching skills.G) Ability to utilize Netsmart to document the tasks required. H) Ability to produce quality work in stressful situations; tackle tough assignments. I) Possess independent decision-making skills. J) Ability to maintain professional interpersonal working relationships. K) Ability to maintain confidential files and matter.L) Ability to independently plan and organize work.M) Ability to establish and maintain effective working relations with supervisors, co-workers, and staff working for outside agencies. N) Ability to prioritize workload and be self-directed. IV) Internal & External Customer ServiceA) This position requires internal and external contacts (community resources and organizations, clients, families and the general public, public/private providers, state office staff, auditors, accreditation organizations, other state programs like DFPS (Department of Family and Protective Services), TSHS, TDHS, etc.) The employee will provide these using written and/or verbal skills, in person and/or by telephone.B) Must demonstrate, compassion, emotional control, and safety practices including suicide safe care.V) TravelA) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.B) Minimum of 80% local travel using personal vehicle. In addition, travel out-of-area for training and meetings. C) Occasionally agency vehicle could be utilized. VI) Equipment UsedA) Utilized as required for position.Lifting Requirements 15-25#Minimum Requirements VII) Minimum QualificationsA) Minimum Education: Bachelor's DegreeB) Defined Education: Health/Human ServicesC) Preferences: Health/ Human Services Acceptable degrees include Applied Behavior Analysis, Behavioral Services field, Communication Disorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health/Human Services, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services field, Social Work, Sociology, and Special EducationD) Substitutions: E) Years' Experience: One (1) Year as an HCS SCF) Defined Experience: Working directly with Individuals with IDDG) License/Certifications: Valid Texas Driver's licenseH) Special Courses: N/AI) Supervisory Experience: N/AVIII) Agency RequirementsA) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.C) Assigned work hours may change as the needs of the agency change.D) The Functional Title of this position may change as the needs of the agency change.E) All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate. G) Employment is at will, as well as agency needs may change.H) Agency dress code is to be always followed.I) Physical on-site presence, including regular attendance and punctuality, is an essential function of this position.
