Job DescriptionPOSITION OVERVIEW: (Primary Purpose of Position) - Post-Acute Care Staff Nurse provides culturally competent, patient-centered, professional nursing care with dignity, compassion, and respect to the diverse resident population, recognizing that our facilities are our resident's unique home-like environment. Under supervision, provides quality clinical nursing care to residents, in alignment with regulatory requirements for long-term care.
ESSENTIAL FUNCTIONS: (List in order of importance to role and time spent on function)• Interacts with all persons (including residents, family members/visitors, workforce members, volunteers, vendors, and others) based on the organization's ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence).• Provides services requiring substantial and specialized nursing skills, following the plan of care signed by the physician.• Develops, implements and continuously evaluates the nursing care plan for assigned residents.• Adheres to the nursing process when providing care/services to residents (assessment, nursing diagnosis, planning, implementation, and evaluation).• Formulates and implements comprehensive, person-centered nursing care plan. Sets priorities, delivers and directs care to meet resident needs.• Develops, implements, and continuously evaluates the nursing care plan for assigned residents. Performs assessment of resident in collaboration with CMS Minimum Data Set (MDS) regulatory requirements which encompasses, but is not limited to: cognitive patterns, mood, behavior, preferences for routine activities, functional status, functional abilities and goals, bladder and bowel, active diagnoses, health conditions, swallow/nutritional status, oral/dental status, skin conditions, medications, special treatments/procedures/programs, restraints and alarms, participation in assessment and goal setting, hearing/vision/speech, and all other care nursing care within the scope of practice.• Records and maintains nursing care plans and progress notes on residents to ensure continuity of care.• Ensures that medical orders are accurate, implemented, and evaluated based on facility policy/procedures.• Document care and services rendered are recorded in the medical record in a timely and accurate manner.• Evaluates residents' responses and makes appropriate changes on the care plan, on an on-going basis. Maintains accurate documentation as described in policies and procedures.• Interprets and reports responses of residents to supervisor and/or members of the health team as necessary.• Collaborates with other members of interdisciplinary team in the care of residents, including the resident and/or family during admission, quarterly, annually, discharge, and significant changes. Discusses education and discharge planning issues as they arise.• Fosters an environment that is safe, caring, and promotes the well-being of residents, encompassing kindness in every interaction, including abuse/neglect prevention, person-centered care planning and delivery, and serves as an advocate for resident's/family needs.• Identifies signs of abuse/neglect/mistreatment/misappropriation of property in this population, including the reporting requirements based on regulations defined in the policy.• Understanding common conditions of the geriatric and frail population with special attention to memory care, functional, and other deficits in a person-centered approach.• Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel.• Provides teaching, guidance and direction to allied nursing personnel in rendering resident care. Ensures allied nursing personnel and licensed practical nursing staff provide care based on the resident's plan of care and document timely/accurately.• Communicates with residents and their loved ones/healthcare representative(s) in an effective and compassionate manner, including resident change in condition and other updates.• Communicates with workforce members, including supervisors, through team meetings/huddles to disseminate information routinely.• Knowledge of Article 28 annual regulatory survey process. Able to articulate with surveyors during survey process.• Maintains professional competence through participation in continuing education and other appropriate learning experiences. Completes all mandatory training/in-services that are assigned in a timely manner. Seeks career growth through Post-Acute Care applicable certifications.• Participates in the mandatory Nurse Residency Program (NRP) during the first year of employment to improve critical thinking skills and evidence-based practice.• Performs other professional nursing activities as delegated by nursing service administration.• Demonstrates safety awareness and follows protocols for fire, disaster, adverse events (code management, elopement), infection control, and other safety practices.• Assists in the orientation/training of new/novice nursing staff through unit-based education and competency. Completes all training documentation timely and accurately for assigned new/novice nursing staff.Participates in quality assurance performance improvement activities, Shared Governance, Clinical Ladder Program, Safe Patient Handling, Pathways to Excellence, and other committees as assigned.
Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).Special Note:For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).THIS POSITION IS PART-TIME, 24 HRS PER WEEK, 12 HR SHIFTNYC Health and Hospitals offers a competitive benefits package that includes:
