  • Reports directly to the Head Nurse/Charge Nurse.
  • Assess patients based on:
  • Interview of patient and/or significant other
  • Chart review.
  • Referrals from other health team members and outside agencies.
  • Collects data on patients receiving care in the clinic and assists MD with all examinations.
  • Educates and teaches patients based on diagnosis and need.
  • Initiates appropriate nursing interventions and update medical record via progress note as needed.
  • Provide counseling and teaching for clinic patients and their families/significant others who are new or referred by the physician.
  • Provide the appropriate nursing interventions based upon assessment, which will include:
  • Instructing patients and/or family members/significant others regarding treatment, medication, diagnostic procedures and the importance of follow-up visits.
  • Interpreting the patient's understanding and emotional response to their condition.
  • Observing and identifying underlying and contributing factors/concerns, especially socio-economic, cultural, physical, and language barriers.
  • Assisting patients and/or significant others to establish a medication schedule when on multiple medications.
  • Making referrals to other agencies as appropriate, i.e., Visiting Nurse Service, Department of Health, Social Services, Home Care, etc.
  • Triage patients who present to clinic without appointments.
  • Make appropriate referrals
  • Communicate with physician or other health care members as indicated.
  • Reports findings to Head Nurse/Charge Nurse as indicated.
  • Document all observations on one or more of the following:
a mt1a ursing ssessment
  • Patient/Family Education Form
  • Nurse Intake Screening Form
  • Continuation/Progress Note
  • Pain screen and assessment
  • Assist with hospital admission process.
  • Expedite process by communicating with necessary personnel.
  • Support patient and family members.
  • Performs EKG, glucometer testing and IV Cannulation Therapy.
  • Participate in preventive health measures and case findings.
  • Maintain health maintenance status of patient by review of necessary records.
  • Assess for human response to actual or potential health problems at the earliest signs and symptoms and initiate treatment and/or make referrals for appropriate care.
  • Identify the family members and/or significant other of patients with an infectious process and encourage testing and prophylaxis as appropriate,
  • e., tuberculin skin testing, hepatitis screen.
  • Review immunization records to update status
  • Identify those patients who are at risk for certain diseases, which could be minimized with appropriate vaccines, i.e., influenza, pneumovax, all other adult and pediatric vaccines.
  • Conducts health education classes to promote wellness, i.e., self-breast examination, parenting classes, Lamaze classes.
  • Communicate patient care problems/issues via the proper channels.
  • Inform Head Nurse of patients with complicated problems and those who are awaiting admission.
  • Has knowledge of immunization and keep abreast of DOH changes.
  • Demonstrates proficiency in the administration of nebulizer and/or metered dose inhalation therapy.
  • Is able to plant PPD and interpret the results as per guidelines.
  • Ensures a safe and secure environment free of hazards to reduce the risk of injury to patients, visitors, and staff.
  • Participates in in-service department and continuing education program in order to foster growth.
  • Able to perform glucometer, knowledgeable of the glucometer machine use, maintenance and quality control.
Minimum Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
