REGISTERED NURSE - HOME HEALTH SPECIALTY INFUSIONS
LOCATION: NORTH SUBURBS
HOURS: DAYS WITH WKD ROTATION AND 1SUMMER/WINTER HOLIDAY
This position is also eligible for up to a $10,000.00 sign on bonus and subject to all taxes! You must be employed in the exact position at time of payout. Internal team members are not eligible for this sign on bonus at this time.
Major Responsibilities:
- Responsibility for patient outcomes through application of the nursing process involving optimal level of self-care for patient and optimal quality of life for patient, family, significant others.
- 1)Performs holistic assessment on assigned patient according to established procedures including but not limited to: i. Evaluation of home environment ii. Availability of able and willing caregiver iii. Identification of appropriate home care services iv. Reasonableness of expectation of patient's ability to achieve goals
- 2)Develops and adheres to appropriate visit frequencies for patient's plan of care.
- 3)Assesses changes in patient's condition that could be potentially hazardous or life threatening.
- 4)Establishes plan of care reflective of identified actual and potential problems/needs and re-evaluates and updates at least every sixty days and as needed.
- 5)Implements plan of care/nursing care plan and revises accordingly.
- 6)Develops discharge plans for patient and significant others in a timely manner.
- 7)Identifies interventions to achieve desired patient outcomes.
- 8)Demonstrates appropriate nursing skills both evaluative and task oriented.
- 9)Demonstrates accountability for delivery of patient care.
- 10)Evaluates patient outcomes in response to actions of health care team.
- Minimization of patient/family knowledge deficits regarding disease process, patient/care, medications, infection control and specific procedures utilizing appropriate teaching processes as well as sharing professional knowledge base with other health team members.
- 1)Provides anticipatory guidance and teaching to patients and caregivers to optimize quality of life and prevent unnecessary/preventable negative outcomes.
- 2)Demonstrates use of appropriate teaching strategies with patients, families and significant others; documents accordingly.
- 3)Demonstrates teaching strategies with other health team members.
- 4)Participates in inservices and orientation of new employees.
- 5)Demonstrates effective use of teaching plans appropriate to patient care plan.
- 6)Provides preceptorship for new staff nurses learning infusion therapy techniques.
- Responsibility for human relations with patients, families, referral sources, physicians, community services and vendors through the use of appropriate, effective, facilitative interpersonal communication skills.
- 1)Utilizes therapeutic communication skills and effective interpersonal skills with patients, caregivers and significant others.
- 2)Promotes multidisciplinary coordination; conducts multidisciplinary conferences at least once during round of care for each patient.
- 3)Participates with other staff members to create a cooperative working environment.
- 4)Maintains confidentiality of information in the field, as well as in the office setting.
- 5)Communicates patient care strategies and patient problems/needs to relevant coworkers; i.e., conferences with supervisors, HHA's, therapies, relief RN's, etc.

- 3)Demonstrates commitment and actively supports the department and organization/agency goals.
- 4)Maintains and supports all regulatory agency requirements, including: i. Accrediting Organization ii. State Rules/Regulations iii. Medicare Conditions of Participation iv. PRO v. Professional Licensure
- 5)Attend to personal work schedule in a consistent and professional manner according to established agency policies/procedures.
- 6)Demonstrates cost effective measures in nursing practice. i. Maintains appropriate supply of medical equipment in nursing bag. ii. Judicious use of office supplies. iii. Appropriate utilization of medical supplies for patient care use. iv. Maintains nursing equipment of medical supplies for patient care use.
- 7)Utilizes basic safety measures/principles. i. Appropriate infection control practices adhered to. ii. Appropriate disposal of hazardous waste. iii. Assessment of safe home environment.

- Provision of positive role model for other health care team members.

Licensure:
Education/Experience Required:
- Graduate from an accredited school of nursing Minimum 2 years Medical/Surgical experience Critical care experience a plus
Knowledge, Skills & Abilities Required:
- Good communication and interpersonal skills Good documentation skills Good problem solving skills Ability to determine appropriate frequency of visits
- CPR Current licensure in the State of Illinois as a registered nurse CRNI preferred
Physical Requirements and Working Conditions:
- Extensive travelling within geographical boundaries in all extremes of weather. Ability to transport patients is required of this position. Exposed to hazardous materials with respect to patient care. Exposed to widely varying social and environmental conditions in the patient's neighborhoods and homes.
