Job SummaryResponsible for collection and processing Vital Statistics information regarding births and deaths at both Chandler and Good Samaritan Hospitals. Processes and signs off on death certificates and/or birth certificates in the Electronic Death Records and/or Birth Vital Statistics system and ensures physician signature within 5 days of death. Helps physicians access and navigate the Kentucky Online Gateway (KOG) and Electronic Death Reporting System (EDRS) and updates the systems by adding and removing physicians. Works with funeral homes, physicians and families to accurately complete death certificates. Communicates with Kentucky Vital Statistics Office. Interviews the mother/parents of each newborn at the Hospital to apply for the birth certificate and completes paternity affidavit forms for the State. Maintains accurate files of all births at the hospital. Explains how birth certificates and social security numbers are generated to the parents interviewed. Accurately types the Certificate of Live Birth form on all live births and mails them to the Lexington-Fayette County Health Department Vital Statistics Office within 7 days of birth. Cross trains with and assists other HIM divisions with tasks which may include but are not limited to data entry, task management, work queue management, clerical support, chart audits, both ongoing and special projects and other duties as assigned.Skills / Knowledge / AbilitiesGood communication skills, attention to detail, computer knowledge, and excellent customer service skills preferred. Bilingual preferred.Does this position have supervisory responsibilities? No Preferred Education/ExperienceHigh School/ GED.
