Position Responsibilities:
  • Excellent communication skills to effectively interact with patients and interdisciplinary team members.
  • Supportive team member who understands the importance of collaboration and teamwork.
  • Ability to meet the physical demands of providing nursing care, including ambulating patients, transferring patients, and assisting with exercises.
  • Assume responsibility for an assigned caseload of patients, conducting OASIS assessments and making planned home visits according to the Plan of Care.
  • Coordinate with therapists, home health aides, and other team members to ensure consistent and appropriate care for each patient.
  • Share responsibility for on-call duty for patients in emergency situations and on weekends, including home visits as required.
Requirements:
  • Graduate of an accredited school of nursing; BA degree preferred.
  • Minimum of one year of clinical experience, with home care experience required.
  • Kinnser training preferred; Oasis experience required.
  • Current license in good standing as a Registered Nurse in the State of Michigan.
  • CPR certification required.
  • Current Michigan driver's license and/or reliable transportation with proof of automobile insurance of personal vehicle.
