The State Registered Nursing Assistant is an individual who, under the direction of a registered nurse, provides personal care services within the guidelines of the IDT care plan to patients admitted to Community Hospice.RESPONSIBILITIES:- Must function within the policies and procedures of Community Hospice- Provides personal hygienic care to patients in their home setting.- Will make home visits to provide basic care as deemed necessary by registered nurse or supervisor.- Will discuss the need for increased or decreased frequency of visits with registered nurse as deemed necessary.- Performs routine nursing procedures under the direction of a registered nurse. These may include, but are not limited to enemas, catheter care, range of motion exercise, body positioning and alignment, and decubitus care.- May request and deliver needed supplies required for routine care.- May demonstrate use of medical equipment with use of proper body mechanics to family.- Demonstrates proper body positioning and skin care techniques that promote decubitus prevention to patient/family.- Monitors and documents vital signs.- Reports noted changes or family concerns to registered nurse or Director of Homecare Services.- Must demonstrate effective communication skills with team members, contract team members, patients, families, physicians and others in the health care community.- Documents care in a legible and timely manner.- Records equipment and supply use in a proper manner.- May assist in replenishing stock and maintaining supply room in neat and accessible order.- Must attend monthly staff meeting and nursing staff meetings as requested.- Must attend in service programs to maintain certification qualifications.- May assist with housekeeping needs as deemed necessary.- Follows the guidelines for universal precautions when providing patient care.- Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security, risk management and patient privacy rules.- Participates with on-call as necessary.- Performs other duties as assigned.Job Type: Full-timeExpected hours: 40 per weekBenefits:

RequirementsQUALIFICATIONS:- High School graduate or equivalent and must be certified as a home health aide by the State Board of Nursing.- Must be current in health requirements requested by Community Hospice.- Completion of a state approved certified nurse assistant course and test.- One year of experience in patient care position in hospital, nursing home or related health care facility.- Continuing education requirements will be maintained in accordance with the State of Kentucky and Ohio requirements for licensure and/or certification.- Personal qualities of empathy, responsibility, dependability, and intelligence.- Must demonstrate effective communication skills with team members, clients, and family members.- Must have the ability to work independent of direct on-site supervision.- Must be able to participate in the Federal and State Healthcare Programs.- Must be able to pass a Driving Records Check, Criminal Background Check, and a Pre-Employment Drug Screen.Education:
- High school or equivalent (Required)
