DYS is the agency statutorily mandated to provide for the care and supervision of youth committed by the District Court to the custody of the Colorado Department of Human Services. The Division operates fifteen secure facilities that serve youth between the ages of 10 and 21, who are pre-adjudicated, sentenced, or committed. The Division also contracts with numerous private residential and non-residential service providers throughout the State. For pre-adjudicated youth, the Division is also responsible for the management and oversight of Colorado Youth Detention Continuum, a State-funded, locally administered program that provides services to youth at risk of further progressing into the juvenile justice system. In addition to residential programming, the Division administers juvenile parole services throughout the State. The program's goals include: Building on current transition planning efforts; Creating a career pathway through pre-employment transition services; Filling the case management gap for the targeted population; and Supporting collaboration between DVR and the Division of Youth Services (DYS).SWAP is intended to provide employment-related assistance to students with disabilities including physical, emotional, or learning disabilities. SWAP provides opportunities for young adults with disabilities to practice and improve workplace skills, consider their career interests, and obtain real-world work experience that will lead to competitive, integrated employment in today's high-demand industries.Purpose of the position:The purpose of the College and Career Specialist / School to Work Alliance Program (SWAP) is to assist young adults in making the transition from school to employment and/or postsecondary opportunities. This role will provide successful employment or post-secondary outcomes, increase community linkages, and new patterns of service for young adults within all categories of disabilities as well as their general education peers.This position provides case management and direct services to young adults with disabilities who are potentially eligible, applicants of or are eligible to receive DVR services (meeting disability documentation requirements and able to benefit from the program) as well as young adults without disabilities; provides support in developing vocational awareness, career exploration, and experiencing career and employment opportunities to become competitively employed and to achieve successful employment outcomes in an integrated setting.The School To Work Alliance program portion of this position will support these pre-employment and postsecondary transition services and reporting components, serve as a liaison between DVR and the DYS, and provide whole and small group instruction. This position is designed to enhance transition but does not supplant those existing requirements of DYS, DVR, and Special Education.Summarized Duties:
  • Eligibility, Plan Development, & Programmatic Documentation for DVR Eligible Youth: Complete all necessary documentation within timelines identified through the DVR process and/or SWAP Contract appropriately with appropriate information. Acts as a liaison to assist in the understanding of DVR processes as well as decisions that affect them. Assist in completing and gathering further information necessary for DVR eligibility determination and service provision including referral for pre-employment transition services. Complete all necessary documentation identified through the contract. Identify youth in transition and maintain accurate records for internal and external stakeholders regarding the DVR components of the role.
  • Planning and Data Collection for college and career outcome: Provide detailed information- via multiple modalities-to youth, education programs, client managers, student families, and other stakeholders as requested regarding college and career opportunities regionally and statewide while concurrently documenting resources. Design and implement lessons for whole group, small group, and individual youth related to career and/or college/school options; this will include potential weeks-long implementation within the classrooms and cooperative planning. Assure tracking mechanism for youth served and topics addressed. Investigate and document information for scholarships, entrance requirements for colleges or trade schools, and other opportunities while monitoring and helping with processes to access these resources. Provide tracking and information for ICAP implementation while meeting QAYS standards related to ICAP. Gather required data as determined by the DYS education office and overall programming to include applications for various things and community resources identified.
  • Providing services/Service Provision: Providing direct service to DVR-eligible individuals and potentially eligible students with disabilities in small group, one-on-one, and classroom settings. Coordinating the delivery of services as identified under an IPE and all IPE Amendments to attain positive program results. Develop and provide pre-employment transition services that support and enhance the transition continuum to potentially eligible students with disabilities. Provide case management linking students to community resources that promote their success. Acts as a liaison to assist in the understanding of DVR processes, decisions that affect them, and methods to appeal decisions if there is a disagreement
  • Providing services/Service Provision: Providing direct service to DVR-eligible individuals and potentially eligible students with disabilities in small group, one-on-one, and classroom settings.   • Partnerships: Develop and participate in interagency transition practices and procedures that result in the creation of viable career pathways that lead to successful employment outcomes for students and youth with disabilities. The position involves interacting with local education staff, community system providers, community referral sources, and DVR staff to promote vocational rehabilitation services.
  • Other Duties As Assigned: Perform other duties as needed for the education programming and needs of youth within the education program including substituting within the classrooms, assisting with school functions, and working with the education and facility teams.

Conditions of Employment:
  • Must be 21 years old or older.
  • Must possess and maintain a valid unrestricted drivers license.
  • Must have no felony convictions and no violent history.
  • Must pass a Colorado Bureau of Investigations and a Federal Bureau of Investigations background check as a condition of employment.
