Substance Abuse Counselor (Per Diem)

Provides chemical dependency counseling, intervention and discharge planning as a member of an interdisciplinary team to adult individuals with chemical dependency and/or mental health problems. Screens potential individuals for admission via telephone/walk-ins to ensure appropriate level of care, using the OASAS LOCADTR tool. Refers individuals to other referral services if the individual is not appropriate for the programs level of care or if there is no bed availability. Receives and processes individuals through the admissions process based on unit admission guidelines, including unit orientation, fire safety, dignified contraband search, confidentiality, alcohol, drug, tobacco free policies, and proper adherence to infection control and hygiene standards. Collects concise, accurate individual Bio-Psychosocial information, completes the Admission Assessment intake form and documents the admitting note.
