Screening 1. Performs annual vision assessments/screening for Pre-school, pre-Kindergarten, first grade, out-of-state, Transfer students and by special request for any student 2. Performs BMI Screenings on Kindergarten students 3. Offers screening services to private schools in the county 4. Records screening results on individual Permanent Health Record, WVEIS electronic health record, and On Sweep Screening Log as applicable B. Referrals 1. Makes referral to the appropriate professional and/or Agencies 2. Refers students with IEP Plan to Special Education Nurse C. Assessments 1. Conducts annual Immunization Survey of all first time enterers as requested by the West Virginia Department of Heath 2. Conducts annual Health Needs Assessment and provides intervention and follow-up as appropriate 3. Conducts individual student assessments: a. physical illness or injury as referred by student, parent or teacher b. behavior assessments as behavior may be influenced by illness or medical condition c. environmental assessment - may include home visits and/or assessment of problems with family interaction D. Records 1. Maintains Permanent Health Records (electronic and file) for all students 2. Maintains confidentiality with student records 3. Completes Incident and Illness Report Forms as Appropriate E. Care of Incidents and Illnesses 1. Orders and stocks first aid supplies for all schools Assigned 2. Carries out appropriate first aid procedures 3. Encourages school personnel to complete American Red Cross Standard First Aid and CPR F. Health Education 1. Teaches procedures from the Basic and Specialized Health Care Procedure Manual for West Virginia Public Schools to appropriate personnel 2. Teaches personnel in disease prevention, detection and treatment as appropriate 3. Serves as a resource teacher on health topics in Classrooms 4. Contributes to personnel in-service programs on health Topics 5. Utilizes community and parent-teacher meetings, hand- of school health and as a means of establishing community rapport G. Healthy School Environment 1. Maintains knowledge of current standards and laws for a healthy and safe school environment 2. Reports laxity in cleanliness, safety, or healthy environment first tot the principal and if not corrected, to the principal's designated supervisor H. Communicable Disease Control 1. Interprets communicable disease laws and policies to school personnel, parents, and the community 2. Assists with the exclusion and readmission of pupils with possible communicable diseases in accordance with State school law and local school policy 3. Interprets to school personnel, parents and the community the immunization regulations in accordance with the West Virginia immunization laws I. Student Support Services 1. Cooperates with other student support services for the general welfare of the students 2. Assists with health-related attendance problems as Needed 3. Works closely with personnel from other agencies and makes home visits as needed 4. Provides follow-up on recommendations for student health/medical assessments 5. Participates in the Eligibility Committee meetings and the IEP or 504 committee meetings as appropriate J. General 1. Assumes other responsibilities as designated by the County Superintendent 2. Follows county policies and procedures specific to areas of employment and school health 3. Evaluates programs, policies, and procedures on a regular basis and makes revisions appropriately 4. Reviews School Nurse Job Description annually and makes revisions as necessary to reflect changes in school nursing standards, school law, and county policy 5. Completes Medicaid billing for appropriate nursing services in accordance with state law. K. Professional Responsibilities 1. Attends professional development meetings, workshops, and seminars relevant to school nursing L. Student Support Services 1. Works cooperatively with school personnel, students, parents, and the community in planning and delivering school health services for exceptional students M. Performance Responsibilities 1. Assists other county school nurses with sweep screening as available 2. Completes evaluations as requested by the School Assistance Team (SAT) 3. Attends Eligibility Committee (EC) meetings and Individual Education Plan Committee (IEP) meetings relative to health issues 4. Develops and implements the Health Care Plans for Special Education students identified as having a chronic health condition 5. Writes the health concerns component of the IEP 6. Acts liaison between school, physician and other health Providers 7. Organizes and provides for the instruction of (with assistance from other qualified instructors) American Red Cross Standard First Aid and CPR 8. Keeps records on Standard First Aid and CPR Training for personnel 9. Requests purchases and stocks supplies as appropriate for students with special needs 10.
