The School Code requires applicants seeking employment at a school in a position requiring direct involvement with children or students to authorize an employment history review as part of the application process. To meet this requirement, please visit www.Verifent.com.
Please note: you do NOT need to pay the $15.00 fee for the Premium option unless you want access to your completed form forever. The FREE option gives you access to your form for 14 days once it is completed (in case you want to print it out for your records). Prior to completing the information on the Verifent website, if you have worked in a position (current or previous) where you were in direct contact with children, please contact your previous and/or current employer(s) that you have worked for to confirm the person/email address the Verifient Verification Requests should be sent to. Once you are on the website, click the 'Let's Get Started Now' button in the upper-right hand corner and select the 'Employees pop-up option. On the next screen, scroll down and select the 'Initiate Experience Verification' button. Please note, if you did not have direct contact with children at your current and/or previous employers, you can disregard Step 3. on the attached handout from Verifient for those employers. For your position, we DO NOT need the Certified Experience Form, so please click the radial button to No. You will need to complete the Faith's Law Self-Disclosure Form. To further help you complete this process, I have attached a step-by-step handout from Verifent for your convenience.
