Job Description PURPOSE OF POSITION Under the general direction of the Director of Nursing, assigns, plans, implements and evaluates on one or more units delivering care for patients. Serves as a role model and assigns, directs and instructs nursing personnel in all clinical areas at all sites in nursing activities. Schedules, assigns, guides, directs and instructs professional and allied nursing personnel in nursing activities. Summary of Duties and Responsibilities: Nursing Process • Monitors and evaluates nursing care through assessment, planning, intervention, and reevaluation, via written documentation, observation, and comparison to established standards. • Reviews and analyzes trends pertinent to the delivery of nursing care within the clinical services at all sites and initiates appropriate innovations or makes recommendations for needed changes. • Assesses and monitors patient's conditions and initiates appropriate nursing intervention including direct patient care. • In coordination with the Director of Nursing, coordinates, guides, and directs the implementation of new/special projects for improving the delivery of care and evaluates as needed. • Assists nursing personnel in organizing patient care and ensuring continuity of care based on knowledge of clinical nursing needs of the patient. • Coordinates and plans with nursing personnel medical/nursing management of patient care. • As a member of the health care team, participates in pre-visit planning, daily clinical huddles, and other activities to meet Patient Centered Medical Home Standards. • Identifies Care Gaps and works with the team to close gaps through proactive outreach, patient education, referrals, and needed services. • Participates in coordination of care activities through Accountable Care Organizations (ACO) and in the development and implementation of other patient care/health care delivery initiatives. • Adheres to standard work flows and completes EPIC work queues as identified. • Maintains clinical expertise in specialty and is available as a resource person for nursing personnel and other health team members in all sites • Assists nursing personnel in assessing patient/family status in order to plan care. • Reinforces quality assurance standards and programs. • Initiates and/or participates in activities including chart review to enhance nursing care and practice in all sites and clinical areas. • Plans, organizes, revises, conducts and evaluates didactic and clinical orientation programs for new nursing staff, supervises clinical experience of nurses during orientation period; makes recommendations for follow-up orientation when appropriate for all sites. • Identifies service/area needs by observations, monitoring and evaluations. • Takes appropriate steps to resolve problems and address staff needs. Leadership/Management of Unit • Provides supervision to assigned areas at all sites. • Serves as a resource person to unit staff and assumes responsibility for the management of several units at all sites, as designated. • Provides leadership, guidance and direction in accordance with departmental goals and objectives. • Promotes open communication within unit, department, and with other services. • Makes sound judgements based on objective information. • Applies principles of problem-solving. • Applies principles of delegation. • Foresees potential problems/situations and intervenes to offset adverse impact. • Provides input in formulating standards of care policies and procedures for department. • Collaborates with administrative support groups to ensure availability of equipment/supplies. • Plans staffing patterns to meet patient acuity level needs. Responsible for monthly nursing department schedule. Notifies superior when unable to maintain continuity of care. • Consults with superior regarding situations needing specific attention or follow-up. • Reviews and approves payroll time sheets for assigned staff. • Offers criticism in a manner that fosters change and more positive productive behavior. • Counsels employees as necessary, takes appropriate disciplinary action for infraction of policy. • Completes performance evaluations according to established policy (keeps anecdotal notes). • Establishes an environment conducive to meeting educational needs of unit personnel. • Provides direction for the staff in the education and teaching efforts directed at patient/family. • Develops, plans, assists in the preparation, implementation, evaluation of training for nursing personnel and educational activities for professional and allied nursing personnel at all sites; • Circulates schedules and individually communicate information regarding in-services, classes, conferences and courses; • Maintains orientation and educational records of programs presented/attended; submits reports in timely manner. • Possess strong computer skills along with the ability to educate users on computer. • Assists in the recruitment, interview selection and orientation of new staff. • Demonstrates leadership ability in the assessment of the environment and intervenes when appropriate to enforce productive working relationships. • Acknowledges accomplishments of staff and other health team members through written and verbal communication. • Determines attainable goals in the delivery of optimal, achievable patient care. • Conducts aspects of supervisory role in fair, consistent, and objective manner. Safety Awareness • Assumes responsibility for safety and comfort of patients, as well as the health and safety of employees. • Participates in and may facilitate daily administrative patient safety huddles. • Collaborates with support services to provide a safe, clean, and secure environment. • Assists in setting and evaluating nursing department standards for emergency equipment, procedures, and disaster plans. • Demonstrates competence in emergency procedures. • Provides for orientation and practice of all emergency procedures. • Reinforces policies and procedures regarding infection control. Self Development • Assumes responsibility for own personal continuing education and developmental needs. • Attends professional meetings, workshops, and conferences to enrich knowledge, and to improve administrative and clinical skills. • Serves on professional, nursing, and facility-wide committees. • Develops competence as resource person, patient advocate and clinical role model for nursing personnel. • Demonstrates resource capability through use of other disciplines. • Accepts constructive criticism regarding nursing action and judgement • Acknowledges that personal behavior influences co-worker's behavior. • Seeks assistance as needed from nurse managers, peers or others in solving clinical or management problems. Personnel Responsibilities • Reports on time and ready to work at start of the shift, complies with time and leave policies. • Observes facility policies governing conduct at work such as telephone usage. • Accepts reassignment to other units and schedules when necessary. • Provides direct care to patients in all clinical areas as needed. • Observes facility uniform/dress code. • Wears identification name badge while on facility premises. • Conduct one's self in a professional and respectful manner while on duty. • Customer service behavior and attitude. • Completes annual mandates and annual in-services. • Conforms to policies governing patient rights and confidentiality of patient information. • Performs other duties as may be requested by Nursing Administration. • Work/or maintain EPIC work ques. • Adherence to wearing allocated uniforms. • Evenings/or weekend assignments as needed. • Follow standardized work for Treat to Target. Minimum Qualifications 1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and 2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and 3. Three (3) years of experience as a registered nurse. Department Preferences QUALIFICATIONS FOR THE JOB:CERTIFICATION(S)/LICENSE(S): • NYS RN License • Certification in Ambulatory Nursing preferred EDUCATIONAL LEVEL: • Master's degree in nursing or other health related field required • Demonstrated commitment to continued professional development.
