RESPONSIBILITIES
- Implement and administer career and transition services including but not limited to developing individualized plans, coordinating placement and transportation and monitoring progress.
- This is a statewide position, the person in this role would be traveling 50-75% of the time.
- Assist in designing and implementing transition programs and services for students in special education, focusing on preparing them for life after high school.
- Support the development of individualized transition plans as part of students' Individualized Education Programs (IEPs).
- Ensure alignment of programs with state and federal regulations (e.g., IDEA, Section 504).
- Lead and manage a team of transition specialists, educators, and support staff.
- Provide guidance and mentorship to teachers, transition specialists, and paraprofessionals delivering transition services.
- Provide professional development and training on best practices in transition planning for teachers and staff.
- Collaborate with staff to ensure consistency and effectiveness in service delivery.
- Establish and maintain partnerships with community organizations, vocational programs, employers, and post-secondary institutions.
- Facilitate interagency coordination to provide seamless support for students transitioning to adulthood.
- Serve as a liaison between schools, families, and external agencies to ensure consistent communication and support.
- Monitor and evaluate the effectiveness of transition services and make data-driven improvements.
- Maintain records, track student outcomes, and prepare reports for internal and external stakeholders.
- Collect and analyze data to assess the effectiveness of transition programs.
- Assist IEP teams in developing individual student graduation pathways.
- Stay informed of emerging best practices and updates in special education and transition planning.
- Ensure compliance with legal requirements, including those under IDEA and other special education laws.
- Oversee the acquisition and utilization of tools, materials, and technology to support transition programming.
WORK SKILLS
- Ability to work independently, prioritize, and manage multiple remote tasks effectively.
- Strong critical-thinking skills, able to anticipate needs and address issues proactively.
- Adaptable to changing priorities, workflows, and technology in a remote environment.
- Proficiency in MS Office Suite, Google, and other collaboration tools.
- Strong verbal and written communication skills, able to correspond effectively across remove and in-person settings.
- Strong attention to detail, independent multitasking abilities, and efficient time management.
- Positive, professional, and capable of collaborating across remote and in-person environments.

QUALIFICATIONS
- Bachelor's degree in Special Education, Educational Leadership, Rehabilitation Counseling, or a related field required.
- Master's degree preferred.
- Active PA Instructional Special Education certification highly preferred.
- Minimum 5 years of experience in special education, transition services, or related areas.
- Demonstrated success in program development and leadership in an educational or community-based setting.
