Job Description:

Nuvance Health has a network of convenient hospital and outpatient locations - Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York - plus multiple primary and specialty care physician practices locations.

Summary:

Provides management responsibilities for the HIM Department to support the Director of HIM, with a major focus in the management and coordination of the coding functions of the Department. In addition, provides comprehensive management of the CRM section. Responsible for planning and implementing the clinical resource management program to meet the needs of the hospital. Provides education to medical staff and hospital staff as appropriate. Facilitates resource management and CQI activities. Acts as resource person to department managers, hospital staff and medical staff. Provides comprehensive clinical resource management for patients. Coordinates patient care to improve quality of care through the efficient use of resources and thereby enhancing quality, cost-effective outcomes. Works closely with the patient, nursing and physician staff to restore health to the patient.

Responsibilities:

* Collaborates and partners with physician and patients regarding plan of care based on age specific needs.

* Routinely reviews the course and outcomes of treatment with the physician and recommends adjustments as appropriate.

* Reviews the patient's response to interventions and variance from expected response with key members of the care team.

* Identifies and facilitates options and services to meet the healthcare needs of the patient.

* Formulates a case management plan based on the needs of the patient; re-evaluates as needed.

* Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, new problems etc.

* Promotes a timely flow of information among and between disciplines.

* Monitors and evaluates the progress of the patient. Collaborates with healthcare team to revise the case management plan when changes occur.

* Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition.

* Works with physician in documenting medical care to accurately reflect intensity of service and severity of illness.

* Facilitates patient access to community resources as appropriate.

* Interacts professionally with patient/family and involves patient/family in the formation of a plan of care.

* Maintains current knowledge of regulatory and accrediting agency standards.

* Uses appropriate reporting mechanisms as required by state laws.

* Able to compile patient data and prepare outcome analysis.

* Serves as a patient and family advocate.

* Addresses and supports cultural practices, as long as such practices do not harm others or interfere with the planned course of medical therapy.

* Provides educational resources to the patient/family as appropriate.

* Serves as a clinical resource person to staff and physicians.

* Maintains a good working relationship within the department and with other departments.

* Manages all coding activity by the HIM Department coders, i.e. maintenance of daily productivity standards, process, changes, workflow.

* Establishes and monitors adequate coding hours to maintain coding productivity standards.

* Provides team leadership for the coding and CRM sections of the Department.

* Educates physicians and other health care practitioners to improve accuracy of chart coding; advises them regarding proper code selection, required documentation, procedures, etc.

* Provides reports regularly to the Director of HIM and the Administrative team related to the CRM and coding sections, i.e. on productivity, physician queries, ALOS, opportunities, etc.

* Assist Director of HIM in preparation and maintenance of the Departmental budget.

* Performs performance reviews (introductory and annual) on appropriate staff members.

* Acts as a liaison to Patient Financial Services, as well as between coding and clinical staff.

* Communicates appropriately and clearly to staff and physicians.

* Facilitates effective patient care and professional practice through continuing education, consultations with other case managers, professional societies and journals and research.

* Demonstrates knowledge of current methodology and practices. Maintains awareness of changes in the regulations and requirements by accrediting bodies.

* Provides supervisory oversight for the nurse audit program, including NHR.

* Demonstrates awareness of the responsibilities of the position and how it interfaces with the rest of the healthcare team. Works closely with all department members as required, is flexible to meet the needs of the department and changes in the workload.

* Interviews and recommends for hire to the Director of HIM staff members for any openings in his/her sections.

* Performs 90-day and annual evaluations on staff under his/her supervision 30 days prior to anniversary date of hire.

* Trains, orients and in-services on department processes, within 30 days of employment, all new employees.

* Communicates regularly with the Director of Health Information Management Services regarding any problems, issues, concerns, opportunities and plans in his/her sections.

* Works with Director/Administration/Revenue Cycle Team to establish comprehensive chart audit programs, including billing, documentation, and coding.

* Performs performance improvement functions through data collection and documentation review.

* Other duties as assigned.

Requirements:

* Bachelor of Science Degree in Nursing preferred.

* Current state Registered Nurse Licensure in Connecticut.

* A minimum of 5 experience in an acute care facility.

* Supervisory experience required.

* BCLS certification.

* Knowledge of ICD-9-CM and CPT coding
