Are you living your life's work?**Entity: Clinical Care Associates****Department: Penn Partners in Care 2****Location: Penn Medicine Radnor- 145 King of Prussia Rd****Hours: Full-Time****Job Summary:**The Supervisor, Patient Coordination (Administrative Lead) for the Penn Partners in Care Program manages the non-clinical Patient Coordinators - Care Management (Care Partners) and collaborates with the Lead, Care Management (Clinical Lead) to support the coordination of the PCSL comprehensive care management program. The role includes hiring, onboarding and management of the Care Partner staff. The Administrative Lead collaborates with internal entities (hospitals, home care, etc.) to develop and improve processes that connect patients to the Primary Care Practices. The role is responsible for the daily oversight of activities across several primary care practices and involves interaction with the entire care team to deliver Care Management services to patients with medically and/or socially complex needs. The Administrative Lead collaborates with the Practice Management and PPC Leadership Team to coordinate and improve care management workflows and identify opportunities to further develop or improve services.**Accountabilities:**Management Responsibilities· Collaborates with Program Leadership in developing an orientation and training program for the Care Partner role.· Hires and on boards the Care Partners.· Provides daily oversight of staffing, schedules and payroll for the Care Partners.· Uses daily reports to help organize and delegate work.· Responsible for performance reviews and performance improvement for the Care PartnersProgram Coordination· Collaborates with the Clinical Lead & Program Manager/Director to develop and improve workflows and processes to support the Care Management program.· Interacts with hospital, home care and IMPaCT program managers to assure effective hand-offs to the Care Partner.· Interacts with practice managers across the region to assure coordinated and efficient delivery of care management services.Quality Improvement· Participates in committees or workgroups to support the improvement and development of the care management program.· Runs performance and volume reports for the team.· Reviews and analyzes program data to assist in program evaluation.· Uses quality improvement strategies to identify opportunities to improve services.Administrative Responsibilities· Oversees payer initiatives that pertain to care management.· Monitors the Regional budget on a monthly basis.· Responsible for ordering supplies for the Regional Care Management TeamPerforms duties in accordance with Penn Medicine and entity values, policies, and proceduresOther duties as assigned to support the unit, department, entity, and health system organization**Qualifications:**· Bachelor's Degree (Area's of Study: Science or Social Work) (Required)· 3+ years of relevant healthcare experience. 7+ years of experience could count as proxy for completed degree.
