I'm respected and my opinion is important and valued - Agrace Employee, 2023 Employee Opinion Survey HOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)The RN Supervisor - Resource PM/NOCcoordinates direct and indirect patient care using an interdisciplinary team approach. The RN Supervisor serves as the functional leader overseeing the shift and directly supervises staff and makes visits as needed. This position is responsible for allocating staff resources based on patient acuity and level of need within the clinician's scope of practice, ensuring quality and continuity of care. This position serves as a resource to team members.
- Resource for Staff
- Collect and utilize data through triage communication, discussions with visit staff and home visits of the patient/family's physical, psychosocial, spiritual and symptom management needs.
- Provide direct care when necessary, as outlined in the individualized Plan of Care (POC) and update the POC accordingly.
- Ensure timely and accurate scheduling and assignment of visits requiring clinical support.
- Identify, interpret, and document the relevant assessments collected in the triage assessment, physical assessment, and other health data.
- Provide support and guidance to direct care staff in helping patients achieve goal outcomes.
- Demonstrate critical thinking skills sufficient to support development of reasonable and achievable outcome measures and establishment of priorities in the planning and provision of care.
- Articulate, identify, and reports signs and symptoms of client abuse and/or neglect, suicide ideation or violence, and risk for pathological grief.
- Apply symptom management expertise according to Agrace standards, policies, and practices.
- Provide support and guidance to team members regarding changes to plan of care and management of complex patients and families. Update and communicate with homecare teams, triage, and leadership as necessary.
- Collaborate with homecare teams, and other disciplines to ensure continuity of care and provide optimal service to patients and families.
- Recognize importance of and maintain professional boundaries.
- Provide supervision, coaching, and mentoring
- Provide direct supervision to any staff working on the PM/NOC shift including but not limited to coaching and mentoring, chart reviews, progress reviews, skills assessments and annual performance evaluations.
- Maintain accountability for approving time sheets and PTO requests; monitor overtime usage.
- Assess educational needs of team; facilitates the necessary development and incorporates knowledge into current practice.
- Participate in the recruitment, selection, and retention of the best-qualified staff; assign preceptors and actively participate in the orientation of new staff on team.
- Establish direction and expectations for staff
- In collaboration with homecare leadership, provide input on the future direction of the Resourceteam in alignment with the overall organizational goals.
- Collaborate with Quality Department, Resource Manager and Homecare Managers to respond to immediate patient and family needs and concerns in a timely manner.
- Utilize patient/family satisfaction survey data and other quality tools to identify trends, areas of concern, and process improvement opportunities.
- Responsible for determining the staffing and visit needs for the PM and NOC shifts; allocate resources based on acuity and level of patient/family needs.
- Miscellaneous
- Maintains current knowledge about community, regulatory and health related programs in order to serve as a resource to team members.
- Participates in the education of clinical nursing students, residents, and internal partners as necessary.
- Participates in mandatory educational requirements.
- Meets regular attendance requirements as scheduled.
- May provide leadership support and guidance to 24/7 operations in on-call capacity as needed.

- Associate Degree in Nursing, Bachelor's degree preferred
- Certification/Registration/Licensure: Current, unrestricted Wisconsin RN license
- Two years relevant RN experience, preferably in hospice, oncology or ICU
- Two to three years supervisory experience preferred
- Ability to work regular weekend rotation.
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Must be at least 18 years of age
- Excellent conflict-resolution skills, organizational skills and time management
- Ability to work effectively and cooperatively with staff, board, volunteers, clients and the public
- Demonstrated skill in facilitating decision-making based on an understanding of culture/ethnicity, gender, race, age, and educational background.
- Ability to use data base software, and other computer software systems
- Ability to work with terminally ill in an interdisciplinary approach
- Ability to communicate effectively verbally and in writing
- Ability to be flexible and effectively manage priorities, multiple tasks and meet deadlines
- Ability to effectively perform all essential function and duties of the position
- Ability to manage, lead and supervise direct patient care
- Effective customer service skills
- Ability to maintain organizational confidentiality which includes, but is not limited to, information about patients/families, volunteers, employees, donors and agency processes
- Demonstrates proficiency in medication administration including oral, PR, sub-cuteanous, transdermal, peripheral IV access, use of central lines, implanted ports and picc lines, and continuous sub-cutaneous infusions
- Demonstrates proficiency in administration of blood/blood products including assessment of and proper intervention for reactions
- Demonstrates proficiency in phlebotomy/blood draws
- Demonstrates proficiency in medication administration
- Demonstrates proper body mechanics when lifting, bending, kneeling, pushing, and pulling
- Demonstrates proper techniques in application of Universal Precautions and Sharps handling
- Demonstrates safe and effective use of DME and other medical equipment and supplies
