Catholic Charities is a non-profit organization that offers an array of programs that strive to empower individuals, children and families. Catholic Charities of Buffalo, NY, is the most comprehensive human service provider serving the eight counties of Western New York with dozens of programs at 40+ professionally staffed locations. Our programs include comprehensive counseling services for all ages, basic emergency assistance including operating 8 food pantries, mental health services, chemical dependency treatment, educational and job readiness services, services to older adults, marriage counseling, immigration and refugee assistance and a variety of programs that address prevention and treatment of a number of family issues.

We are hiring a full-time Supervisor (35 hours per week) for Project Hope at our Kenmore location at 3370 Delaware Avenue, Buffalo.

JOB SUMMARY:

Position provides supervision to the Project HOPE program which provides time limited Care Coordination services in the home and community to keep an older adult with a mental health diagnosis independent in their home environment through linkage, referral, advocacy and intervention with formal and informal supports. Position is responsible for direction of daily case management and RN (including referral management, screening and assignment) operations, contract compliance, budget oversight, and direct administrative and supportive supervision of case managers/RN that empower the older adult to remain successful in the environment they deem most comfortable, home.

ESSENTIAL FUNCTIONS:

* Exemplify Catholic Charities' mission, vision, guiding principles and policies and procedures.

* Promote and model adherence to high ethical standards, consistent with the Catholic Charities' Corporate Compliance Program, Code of Ethics, legal mandates & labor laws, and other applicable contractual/accreditation standards.

* Communicate regulations, standards, applicable policies and procedures to team members and monitor compliance. Maintain Project HOPE program policies & procedures and uniform practice standards that align with above standards; Compile in program operations manual (s).

* Recruit, orient, and lead a small, diverse team of case managers. Model and endorse a client-centered, engaging, strengths-based, solution-focused, empowering, and trauma-informed care approach to services. Cultivate a culture of sensitivity and respect for equity and diversity.

* Assist in identifying and implementing best practices with respect to poverty that maximize performance and ensure achievement of program objectives/targets.

* Assist with developing, managing and monitoring related program budget. Designate and monitor staffing allocations to ensure accurate alignment with assigned program/service responsibilities within program budget.

* Routinely utilize 1011 or other database system reports/queries to monitor case manager adherence with timely entries and manage productivity and quality performance. Compile and analyze all data to both ensure quarterly file audits are being completed, and compliance with contract and funder requirements and benchmarks, agency policies and procedures, program tenets.

* Respond (verbally and documented via Incident Report system) to crises and high risk/safety concerns; Identify and implement corrective action as necessary.

* Lead design of and implement a quality assurance procedure to monitor case manager and nurse adherence to program standards, regulations & procedures and service targets/outcomes, including routinely reviewing case records to assure compliance with contractual agreements.

* Contribute to contract-specific reports as directed, as well as periodic, semi-annual and annual report information in alignment with agency, Erie County program and funder standards. Routinely review Erie County reports with program team members, evaluate program compliance.

* Occasionally represent Catholic Charities of Buffalo at community and professional committees, meetings and events, including older adult coalition meetings.

* Actively cultivate and expand strategic, strong and resilient relationships with key stakeholders, referral sources, and service partners, to assure effective coordination of services and collective solution-building. Routinely evaluate stakeholder (service recipients, partner providers, community, etc.) satisfaction and likeliness to promote CCB programs.

* Facilitate professional development opportunities to support staff's skills and expertise to foster quality service provision, including effective utilization of individual and group supervision sessions and home/community safety strategies/ procedures.

* Provide guidance, consultation, and direction to case management/nurse team with screening, assessment and service delivery as needed.

* Conduct provisional and annual performance evaluations as required; maintain performance accountability and implement corrective supervisory action as warranted.

EDUCATION: Bachelor's degree in social work, mental health counseling, gerontology or another approved related field preferred. Master's degree strongly preferred.

EXPERIENCE: Two (2) to four (4) years of related social work experience and demonstrated ability to perform position responsibilities

KNOWLEDGE, SKILLS, and ABILITIES:

* Demonstrated ability to engage, lead and maintain effective relationships with internal and external teams

* Demonstrated agility, initiative, ability to work independently and efficiently (effective organizational and time management skills) and produce results

* Familiarity with Erie and surrounding county older adult services and supports

* Fluent in various computer applications (e.g. Word, Excel), electronic database systems and electronic health records; 10e11 eCR a plus.

* Position demands understanding of poverty and movement toward stability, flexibility, and creativity. Ability to engage with diverse clientele experiencing impact of poverty, short- and long-term multi-problem situations and urgent need.

* The strongest candidates for this position will have experience working with at least one of the following:

* Homeless populations

* Age 60+ older adult with a mental health diagnosis or serious mental illness (SMI) Diagnosis in need of additional support and systems navigation

Special Requirements:

* Requires travel - must possess valid NYS driver license and agency-required level of auto insurance coverage and be able to travel as needed within the eight county WNY region.

* Must be prepared to work a flexible schedule in order to allow for oversight of daily operations, as well as agency related activities/events that take place outside of typical business hours.
