Job Description Supervises and evaluates the nurse liaisons in formulating and implementing a homecare plan; implements policies and procedures of the home care network. Interprets regulatory guidelines for admission to Home Care; collaborates with home care agency and subacute discharge planning staff. Assists with the ongoing promotion of the Home Care Network. Job Responsibility
- Promotes Referrals department goals by selecting, motivating, and training capable team members.
- Leads the activities of assigned Referrals team members by communicating and providing guidance toward achieving department objectives.
- Supervises and coordinates the home care liaison staff and their activities; supervises, hires, trains, disciplines, and evaluates the performance of staff; ensures performance appraisals are completed in a timely manner.
- Collaborates with management concerning personnel problems and any subsequent disciplinary actions; acts as a resource to professional staff regarding patient related issues.
- Assists in the on-going functioning, growth, and development of the home care network; oversees and arranges the scheduling/staffing of nurse liaisons at subacute facilities.
- Assists with data collection and analysis for productivity measurement and analysis of referral/admissions; reports to management issues related to planning services.
- Identifies potential shifts in service care needs or health care trends impacting future planning; promotes awareness of the home care network and its services within the organization and to the general community; assists in ensuring the development of high standards of quality care.
- Participates in performance improvement activities; reports the outcome of evaluation activities of staff.
- Monitors documentation from nurse liaisons for evidence of comprehensive home care planning to ensure adherence to standards of quality, appropriateness and compliance with regulations; schedules regular in-service programs for professionals to enhance their knowledge base.
- Provides information to staff on relevant continuing education opportunities; interprets the home care network's policies and procedures; collaborates with managers and staff of all home care agencies; performs job functions in a fiscally responsible manner consistent with the Network's mission and vision.
- Serves as a participating member on the network's professional committees; takes problem/complaint calls, when necessary, from patients, families, physicians and/or hospitals.

Job Qualification
- Graduate from an accredited School of Nursing required.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
- 4-6 years of relevant experience and 0-2 years of leadership / management experience, required.
