Location: Plymouth,MA, USA
* Positively supports the VNA Telemonitoring program by actively reinforcing telemonitoring as a standard of care when interaction with internal and external customers.
* Works with the Director of Public Health/Wellness and Clinical Strategy to ensure the assignment of telemonitoring as applicable to patient centered/disease management goals of care.
* Ensures clinical outcomes are achieved and necessary interventions are provided by monitoring transmissions of Telemonitoring patients. Communicate appropriate information as needed.
* Ensures detailed, timely documentation of telemonitoring encounters in the patient's clinical record.
* Evaluates the patient/caregiver response to telemonitoring via clinical data analysis and telephone triage.
* Assists with educating clinical staff, physicians and patients about the benefits of telemonitoring in managing disease states and preventing complications.
* Generate Reports per program policy and procedure and as requested for trends and outcomes.
* Participates in Agency marketing activities as assigned.
* Supervise and oversee Telehealth competency of all direct reports and conduct annual performance evaluations of direct reports.
* Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.
* Challenges current working practices identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.
* All other duties as assigned.
* Registered RN or LPN currently licensed in MA.
* Minimum of three years clinical experience required, recent community health experience preferred
* Previous supervisory experience preferred
* Word processing/data entry skills required.
