CAMBA's Initiating Change Around Reproductive Empowerment (iCARE): Through a grant from the New York State Department of Health (MICHC)
, iCARE seeks improve maternal and infant health outcomes for low-income women and their families and reduce disparities in those outcomes, proving support and impacting service systems in the 11203, 11226, 11233 and 11212 communities.iCare community health workers (CHW) assist women and their families to effectively access continuous and coordinated healthcare and other services, including home visiting. CHWs provide outreach, information, assistance with access to services, advocacy, and enhanced social support.
Position: Supervisor (Temp)Reports To: Program ManagerLocation: 21 Snyder Avenue, Brooklyn, NY 11226What The Supervisor (Temp) Does:- Communicate effectively with participants, direct reporting staff, peers, supervisors, and funders.
- Plan and organize program activities to maximize program contract's goals and performance targets.
- Motivate, coach and counsel direct reporting staff to excel.
- Improve teamwork among direct reporting staff as well as among peers.
- Troubleshoot participant and direct reporting staff program problems and make decisions in accordance with program policies, procedures, and protocols.
- Develop program content regarding maternal and child health, reproductive health/wellness and reducing barriers to health care/health equity
- Provide staff training to CHWs on program content on maternal and child health, reproductive health/wellness , family planning including childbirth education, pre-natal health, birth plans, breastfeeding, maternal mental health, postpartum wellness
- Plan, coordinate and facilitate peer support events/groups, community workshops and events focusing on topics of maternal and child health.
- Plan, coordinate and and conduct outreach in order to recruit participants, including developing marketing materials
- Co-facilitate monthly outreach team meetings.
- Monitor staff completion of outreach activity and analyze the effectiveness of the strategy.
- Develop and enforce quarterly outreach plan and schedule and assign staff to different venues.
- Co-Lead initiative to develop Coordinated Intake for HomeVisiting and other maternal-child health programs and support relationships between providers in maternal child health community.
- Overcome resistance to change from participants, direct reporting staff, and supervisors/funders.
- Monitor participants' progress toward their goals and document via progress notes.
- Provide supervision to CHWs to ensure participants' progress towards goals, follow up on service referrals and assessment for high risk issues such as mental health and intimate partner violence/abuse
- Review all documentation related to participants' progress for accuracy, completeness, and clarity (including case records and date entry forms).
- Provide all required information for weekly/monthly/quarterly/semester/annual reports to CAMBA management and/or to funders.
- Immediately report to the appropriate Program Manager any: monitoring visits or funders' events; significant events; any incident that might subject CAMBA to liability.
- Review and sign time sheets.
- Prepare performance appraisals for direct reporting staff.
- Work with direct reporting staff to improve work performance through participant feedback, training, weekly documentation of one-on-one supervision of staff, home visit observations and home visit verification calls.
- Administer constructive discipline to direct reporting staff in compliance with CAMBA's corrective action guidelines.
- Manage own time effectively and coordinate program activities to maximize time of direct reporting staff.
- Support team building and staff retention.
- May prescreen participants over the telephone for eligibility and may schedule intake appointments.
- May conduct initial intake or assessment of participants and/or participants' families and periodic reassessments.
- May have direct participant service/program responsibilities in addition to the above.
- May make hiring and firing recommendations to appropriate Program Manager/Assistant Deputy Director/Deputy Director (in consultation and agreement with Human Resources
Minimum Education/Experience Required:- Licensed Master's Degree in Social Work (ie. LMSW, LCSW) or related field and two years of supervisory experience, or Licensed BA in Nursing (RN or NP) and three years of supervisory experience.
- Experience providing childbirth education and support (including development of a birth plan) and/or pre-natal health education/social support
- Experience facilitating peer support group
- Experience conducting community outreach
Other Requirements:- Some evenings and/or weekends may be required.
- Experience with community organizing or coalition building.
Compensation: $62,400 annuallyWhen salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
Status: Full-time (35 hours per week) (Six month position)
