POSITION SUMMARY: Provides direct support brokerage services on behalf of persons with Developmental and Intellectual Disabilities, in gaining access to needed services, i.e., social, medical, educational, and vocational. Implements guidelines for Medicaid Waiver Programs and Consumer Directed Option Services across the region

QUALIFICATIONS: Bachelor's Degree in Social Work, Psychology, Sociology, or a field relevant to Human Services; or a Bachelor's Degree in Nursing with a current Kentucky License; or a Kentucky Registered Nurse with a current Kentucky license and two (2) years' experience in working with the elderly or disabled; or a Licensed Practical Nurse with a current Kentucky license and three (3) years' experience working with the elderly or disabled.

SKILL SETS: Possesses superior organizational, oral, and written communication skills. Must demonstrate a high level of familiarity with Microsoft Office specifically MS-Excel and exceptional customer service skills.

PHYSICAL AND ENVIORMENTAL DEMANDS: Visual acuity and manual dexterity needed for required paper work and data entry. Frequent talking and hearing required, when communicating with staff individuals, families, and other service providers. Patience and high stress tolerance with ability to make decisions under pressure. Meets potentially violent individuals at their home, hospital or jail with possible risks of potential abuse and unsanitary conditions. Work is light with walking, standing, and occasional stooping. Possible exposure to blood-borne pathogens, bio-hazardous waste, and hazardous waste. Possible exposure to communicable diseases. May require evening, weekend consultation.

JOB SPECIFIC FUNCTIONS:

  • Documents through staff notes monthly face to face visits and monitoring of identified services (service plan, service notes, time sheets, etc.).
  • Communicate effectively through verbal and written form.
  • Secure and monitor services provided to assigned individuals, which may require traveling to various areas.
  • Facilitates, developed, reviews and updates Plan of Care (POC) and CDO support spending plan.
  • Implement effective organizational skills.
  • Evaluates and processes pre-admission packets of individuals to receive Medicaid waiver or DDID funded services.
  • Completes monthly/annual paperwork, monthly billing, and all other needed documentation.
  • Maintain all training requirements in order to stay in compliance with Medicaid Regulations and Communicare policies.
  • Performs face to face in-home assessments of potential Medicaid Waiver recipients.
  • Participates as an active member of the Support team which develops the support plan and/or facilitates for Person Centered Plans for those individuals who receive funding through Medicaid waiver and CDO services.
  • Assists families and/or individuals in crisis situations which may result in admission into a facility to meet their needs.
  • Teaches, trains, and monitors the development of service plans and Person Centered Plans through the support team.
  • Plan, identify and implement support strategies for individuals including principles of empowerment, community inclusion, health and safety assurances, and use of informal supports.
  • Promotes the policy of human rights and attends Human Rights Committee Meetings.
  • Responds by phone in a timely manner to crisis calls and advises caller of possible options.
  • Participates and provides leadership in agency continuing quality improvement (CQI) activities.
  • Performs other duties as assigned.
