JOB SUMMARY The Support Coordinator supports the mission and core values of BDI, Inc. by providing service coordination within the parameters of the Department of Behavioral and Developmental Disabilities (DBHDD) contract to identify, coordinate and oversee the delivery of services that enhance the health, safety, general well-being of individuals living with a developmental disability and lead toward maximum independence. ESSENTIAL DUTIES/RESPONSIBILITIES •Conduct Needs Based Assessments for individuals to connect them with needed services, community resources, and support networks (in-person and virtual). •Lead Individualized Family Support Service (IFSS) plans with participants and families to review and plan services available to participants. •Provide support coordination services to individuals in the program that require essential health and safety services and supports. •Assess, coordinate, broker, plan, and/or link individuals and families to direct services including acting as an intermediary between the individuals and families and the providers or vendors of direct services. •Advocate for participants to obtain needed services or benefits. •Provide transitional support for individuals from the IFSS program to other services once stable. •Authorize and monitor finances for goods and services. •Maintain progress notes and contact with enrolled individual no less than two times per month during IFSS services. Notes should include updates on family situation, noted improvements, barriers, etc. •Facilitate virtual meetings, support, trainings, webinars and presentation of resources (in-person and virtual). •Attends regional field office provider meetings. •Other duties as assigned. This may not constitute an entire list of duties REQUIRED EDUCATION AND EXPERIENCE •One year experience in direct client services. •One year experience in serving persons with developmental disabilities. •BA Degree in Social Work, Psychology, Rehabilitaion Counseling, or related discipline PREFERRED EDUCATION AND EXPERIENCE •BA Degree in Social Work, Psychology, Rehabilitation Counseling, or related discipline. •Certified Rehabilitation Counselor or Certified Vocational Evaluator, preferred. •Licensed Independent Clinical Social Worker (LCSW); or Licensed Professional Counselor, preferred. ADDITIONAL ELIGIBILITY QUALIFICATIONS Must pass background screens. SKILLS AND ABILITIES COMPETENCIES •Good interpersonal skills and the ability to work well across departments. •Excellent customer service skills. •Ability to work independently. •Detail oriented. •Knowledge of software applications. •Excellent problem solving and analytical skills, as well as the ability to gather and evaluate information. •Excellent written and oral communications skills. •Demonstrated ability in report writing. •Counseling and coaching skills.
