The Support Coordinator will be responsible for conducting assessments, developing plans of care, assisting individuals with accessing community resources and monitoring delivery of approved services.A Bachelor's degree in a Human Services field is required. Qualified candidates must demonstrate excellent writing & communications skills. Local travel is required, a valid Louisiana driver's license, auto insurance, and a reliable vehicle are a must. MDS-HC and Plan of Care Certification preferred but not required.
