Job Description:

Hope For Youth's Intensive & Supportive Case Management (ISCM) program is an out-patient preventative service that provides children and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCM's supportive track (SCM), can accommodate the needs of up to 20 children and their families. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County.The ISCM program is designed to help prevent disruption, multiple placements and placements in more restrictive settings. Hope For Youth's caseworkers utilize proven screening tools such as Child and Adolescent Strengths and Needs Screening. ISCM staff work closely with the DSS. Through interdisciplinary assessments, DSS and Child Protective Services provide the Hope For Youth ISCM team with information pertaining to the families past and current functioning so that Hope For Youth's Case Managers can provide recommendations for going forward. Communication between Suffolk County and HFY is ongoing.To be a successful Supportive Case Manager you must be ready, willing and able to:Provide services to maintain child in current living environment and work to achieve positive outcomes/deliverables as outlined in the service contract/grant.Assess and address safety and health concerns in the child's current home environment and provide appropriate advocacy as needed.Conduct twice monthly face to face contacts/home/community contacts, as well as weekly phone contact, and completes all necessary reports, paperwork and forms as outlined in the program operations manual.Maintain a caseload up to 20 clients.Perform all related tasks or other agency and/or programmatic required needs associated with this role, the needs of the clients and programs.Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligationsHow will you generally spend your time in this job:Field work Documentation and related paperworkStaff meetings, committees, supervision, Interdisciplinary Treatment Team MeetingsTraining and staff developmentOther assignments, as assigned.Minimum Requirements that must be obtained and/or maintained:BSW or bachelor's degree in a related fieldValid NYS Driver's License in good standingAccess to a reliable vehicle that can be used on a regular basis for home visits and transports.Excellent writing skills/time management.Ability to be flexible with scheduling (work evenings, possible weekends if necessary).Responding to all calls/emergencies via agency-provided mobile phone and/or via internet technologiesPhysical Requirements: read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving, ongoing access to reliable vehicle and driving own vehicle for workHere are some of the benefits of working for Hope For Youth! •Benefit eligibility commences on the first of the month following the first 30 days of employment•90% Employer-paid medical premiums •Generous paid time off for all employees regularly working over 20 hours •Agency-funded Pension Plan •Pre-tax savings plans: self-directed 403(b), 529, FSA and HSA Accounts •Agency-paid life insurance and disability, and for some positions, long-term disability •Training programs and Tuition Assistance and free tuition credits are also available •Employees may be eligible for Student Loan Forgiveness Programs•Special Events and Recognition Awards •Meaningful work that makes a difference in the lives of children and families of Long Island! •Volunteer and Internship opportunities available within •Supportive like-minded, mission-driven co-workersPlease include resume, cover letter and salary requirements.
