Job Description:

The Supports Coordinator is a key associate at Hope Network whose primary responsibilities include: pre planning, person centered planning, coordination, linking, and brokering needed services and monitoring of all authorized services necessary to maximize the person's served recovery and independence. The Supports Coordinator ensures compliance with agency standards and the practice model, assists persons served to improve their quality of life and self-determination. The Supports Coordinator will implement strategies for obtaining needed services and supports that are goal-oriented and individualized.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: This is not intended to be an exhaustive listing of job functions. The employee is required to follow any other instructions and to perform any other duties as assigned by management. -

Where credentialed, perform comprehensive psychosocial assessments to determine needs and required level and frequency of medically necessary services. -

Conduct ongoing assessments of persons-served and evaluate effectiveness of treatment goals. -

Meet with persons served face-to-face as deemed medically necessary in their Individual Plan of Service. -

Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker. -

Facilitate access to community resources and social support networks for persons-served. -

Provide transitioning and follow-up services. -

Coordinate and ensure the persons served involvement in the Person Centered Planning Process (PCP), self-determination, recovery and relapse prevention. -

Support the engagement of meaningful activities by coordinating and monitoring service activities such as a job, school, volunteerism, family caretaking, for persons-served. -

Participate in legal proceedings including petitioning and testifying in clinical emergencies. -

Promote self-determination and provide referral to appropriate community resources. -

Utilize his/her technical skills and knowledge to analyze and recommends strategies for self-determination. -

Coordination with the Medicaid health Plan, Medicaid fee-for-service, or other health provider. -

Complete and submit Service Activity Logs (SAL's) in accordance with agency policy/protocols. -

Support and encourages the population served to reach their full potential. -

Provide after hours on-call services as required. -

Maintain records, meeting all appropriate standards according to company policies ad all regulatory bodies. -

Educational / Talent Requirements: -

Minimum -

SC -Non-QMRP: LLBSW or BA in Human Services (requires supervision to provide SC services; not providing SC services prior to 2008). -

SC -QMRP-LBSW ( No supervision required ; LLBSW provided services prior to 2008) -

SC -Masters QMRP (No supervision and this individual maintains a Master Degree but is not fully licensed. In some cases, able to perform therapy with the supervision of a MHP. -

Preferred

SC –Masters MHP (Able to provide services with no supervision and is able to complete assessments and perform therapy in addition to supports coordination. - Concepts of Motivational Interviewing - Integrated Duel Diagnosis Treatment - Family Psycho Education - Seeking Safety, Family Psycho Education - Stages of Change - Trauma Informed Care
