Description
  • Supports Coordinators are socially responsible for locating, coordinating, and monitoring services for consumers on their caseloads. They are required to meet consumers in their homes, day programs attended, and in the community. Supports Coordinators will work flexible schedules and use emotional intelligence in order to meet their consumers' needs. Supports Coordinators will follow regulation guidelines by Office of Developmental Programs (ODP) and meet monitoring requirements based on their funding level: SC Services Only, Base, PFDS, Community Living Waiver, or Consolidated Waiver.
  • Supports Coordination is a service that involves the primary functions of locating, coordinating, and monitoring needed services and supports for individuals being served. Supports coordinators are responsible for writing an Individual Support Plan for each consumer and maintaining that plan throughout the year to accurately reflect the individual's life and services.
    • Locating services and supports consists of assistance to the participant and his or her family in locating, arranging, and obtaining services-including needed medical, social, habilitation, education, employment, transportation, and other needed community services.
    • Coordinating consists of development and ongoing management of the ISP in cooperation with the participant, his or her family, members of the ISP team, and providers of service.
    • Monitoring consists of scheduling with the individual in their home, day program setting, or community to ensure the individual service and supports are continued to be implemented as well as, their health and safety needs are being met. Supports Coordinators are responsible ensure resolutions of issues/conflicts founded during monitoring. Supports Coordinators will meet with their individuals with accordance to ODP regulations.
  • Documentation
    • Supports Coordinators are expected to: have a thorough knowledge of required paperwork expectations and complete paperwork requirements within timelines required by ODP.
    • Review and abide by all Community Guidance Center Policies and procedures pertaining to documentation.
    • Complete all paperwork requirements of the department, agency, county and state authorities in a timely manner.
    • Maintain awareness of all federal, state and agency policies and regulations regarding Supports Coordination and perform accordingly.
  • Training and Supervision
    • All Supports Coordinators will be socially responsible to complete the ODP SC Orientation Training within the first month of hire. All Supports Coordinators are required to complete all mandatory courses found on Relias required by the Community Guidance Center. Supports Coordinators will be responsible for continuing to complete required trainings throughout the first year of hire as specified by ODP and will be responsible to complete 24 training hours each fiscal year.
    • Attend monthly department meetings as scheduled
    • Attend a scheduled monthly face to face supervision with the Director using open communication.
    • Consistently demonstrate the interpersonal skills necessary to deal effectively with others using emotional intelligence.
  • Crisis Prevention/Intervention
    • The Supports Coordinator must participate in a 24 hour on call system for after hour emergencies/crisis. All Supports Coordination staff are responsible for participating in weekly rotating on call schedule.
  • Adhere to seven recognized Sanctuary commitments which includes social responsibility, social learning, non-violence, open communication, democracy, emotional intelligence, growth and change.
Position Requirements
  • Bachelors Degree including 12 credits in related social sciences
  • Valid Pennsylvania driver's license
  • Act 33 & 34 Clearances and FBI Clearance and Act 31 Mandated Reporter Training
  • Intellectual Disabilities experience preferred
  • Must demonstrate emotional intelligence in dealing with conflict between staff and/or consumers Must be able to negotiate, navigate, and coordinate the multiple services involved in Referrals and consumer cases
  • Must have and maintain a valid driver's license and vehicle to attend consumer/agency appointments outside the office
