Advanced Behavioral Health, Inc. is looking for a Targeted Case Manager, who will play a pivotal role in providing support and guidance to young individuals facing various challenges such as behavioral problems or suicidal ideation. This position will be responsible for assessing their needs, developing comprehensive care plans, and connecting them with resources and services to promote their overall well-being and success. ABH is a family centered mental health program that offers a wraparound approach to treatment. We specialize in Psychiatry, Off-Site Counseling Services, On-Site therapy, and Therapeutic Mentoring. Location: Gaithersburg, MDPosition Status: Full-Time; ExemptCompensation: $55,000 - $65,000 annual salary based on experienceOffice Hours: Core office hours: 9:00am - 5:00pm; Occasional evening or weekend hours. Travel: Travel is expected for this position up to 60%Essential Functions:Complete comprehensive needs assessments to identify the unique needs and goals of children, adolescents, and their families in crisis. Establish treatment programs by setting schedules, coordinating services, arranging resources, and providing transportation and escort services as needed. Identify and access community resources, including educational, employment, safe housing, healthcare, meal planning, budgeting, and counseling services, to support clients in achieving their goals. Establish partnerships with local organizations and agencies to expand the range of available resources for youth clients. Advocate for clients to ensure they receive appropriate services and support. Bring support services to the clients in homes, schools, or in the community. Coordinate with Crisis Intervention to support youth facing emergencies or challenging situations or risk factors such as homelessness, running away, substance abuse, mental health crises, or family conflict. Develop safety plans and connect clients with immediate assistance and appropriate interventions as needed. Facilitate workshops and individual sessions to empower clients to develop necessary skills for independence such as budgeting, job readiness, communication, conflict resolution, and decision-making. Provide continuity of care and support services in addition to their therapy for up to 6 months under the program. Track clients' progress towards their goals and objectives outlined in their care plans and outcomes achieved. Evaluate program effectiveness and identify areas for improvement through data analysis and client feedback. Ensure compliance with CARF/COMAR/HIPAA/State regulations and uphold EMR documentation standards. Qualifications: Bachelor's degree in Psychology, Social Work, Counseling, or related discipline. Previous experience working with youth populations preferred. Knowledge of clinic territory, community resources, and social service agencies. Valid Driver's License and reliable transportation. Ability to manage, delegate and manage multiple tasks with completing deadlines and priorities. Excellent active listening, communication, and documentation skills. Strong interpersonal and customer service skills are essential. Ability to work independently and collaboratively in a fast-paced environment. Strong organizational and prioritizing skills. Bilingual - fluent in English and Spanish preferred. Empathy, patience, and cultural sensitivity when working with diverse populations and families in crisis. Flexibility to adapt to the evolving needs of clients and program requirements. Working Conditions: This position may require occasional evening or weekend hours to accommodate client schedules or participate in community events. Work may be conducted in office settings, client homes, schools, or community centers. Travel within the local area may be necessary to meet with clients or attend meetings. The employee must be able to transport self and clients to various locations within the assigned counties.
