JOB ANNOUNCEMENTJob Title: TASC Care Manager Location: Remote (must reside in Pitt, Craven, or surrounding counties)Hours: 40 hours/week, Full Time, M-F, Non-ExemptCoastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team. This position offers a unique opportunity to support justice-involved individuals by providing case management and care coordination services within the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will work remotely but collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services.Pitt County, NC (or surrounding counties)Primary duties include administering psycho-social screenings/assessments, coordinating linkage to behavioral health treatment and medical, educational, and vocational services as needed, and providing ongoing professional care management services for the justice-involved population. As a part of this position, the Care Manager will provide telehealth services to counties throughout Eastern North Carolina as needed to support TASC operations. In addition, occasional travel is required within the assigned service area. Qualifications:Education: High school diploma or higherCertifications: Preference is given to individuals with North Carolina Peer Support Specialist certification and at least two years of experience or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB). Experience: Minimum of 2 years of experience in case management, care coordination, or a related role, ideally within behavioral health or substance use disorder services. Experience working with justice-involved individuals or in the justice system is highly desirable. Knowledge of substance use disorders, recovery principles, and available treatment options. Familiarity with the North Carolina justice system, community resources, and TASC programs is a plus.Required Skills: Strong communication, organizational, and time-management skills. Ability to work independently and manage a caseload effectively. Proficiency in using electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.). Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.Salary: Salary Range for this position is $48,000-$50,000 commensurate with experience, medical/dental insurance, life insurance, disability insurance, retirement savings plan/401K, paid time off programs.
