As a trusted community partner, WellLife Network fosters a nurturing environment where everyone has the opportunity to thrive and realize their fullest potential.Position Summary: Responsible for establishing, administering, and directing a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation and support services to identified clients with severe and persistent mental illness; supervise and evaluates staff performance to ensure service excellence and courteous, helpful, and respectful services to program clients; and function as a practicing clinician on the team.Essential Accountabilities:
  • Leads daily team organizational and service planning meetings, provides direction to staff regarding individual cases, and regularly conducts individual and group supervision meetings.
  • Meets required productivity targets and completes all documentation within assigned time frames.
  • Monitors and coaches staff on meeting productivity measures.
  • Directs and coordinates the client admission process and treatment, rehabilitation and support services of the program.
  • Participate in staff recruitment, interviewing, hiring, work assignments and orientation and performance supervision.
  • Develop and implement staff orientation and training.
  • Develop and maintain program policies and procedures and revise as necessary.
  • Perform on-call duty for crisis intervention and for staff supervision and consultation.
  • Oversee and monitor accurate use of internal electronic health record (Avatar).
  • Provide guidance in engagement and communication with enrollees.
  • Provide leadership through positive modeling and be open to the opinions of all team members.
  • Provide guidance to ACT staff for identification of potential barriers to care and resolutions to those barriers.
  • Audit client charts on a regular basis.
  • Coordinate and monitor services with AOT, mental health and criminal courts, probation, and child protection services on an ongoing basis.
  • Responsible for crisis intervention requiring 24 hours on call availability.
  • Provide disciplinary actions to staff in violation of agency, contract or program policy.
  • Act as a clinical liaison between the team and outside entities.
  • Document any progress notes in EHR within 48 hours of all client and collateral contacts.
  • Ensure all documentation is completed in a timely fashion and remain in compliance with agency, OMH, and AOT regulations.
  • Communicate pertinent consumer and operational information to the Deputy Director, Division Director and Regulatory bodies.
  • Participate/Organize case conferences and case consultation to ensure a high standard of service provision.
  • Conduct field based visits. The ACT Program provides 85% of their services in the community.
  • Participate in OMH approved/required ACT Orientations and Trainings.
  • Assists in the planning, developing and implementation of goals and training to further develop participants skills, including but not limited to:
    • Budgeting
    • Grooming
    • Nutritional planning
    • use of public transportation
    • use of community resources
    • Shopping
    • Housekeeping
    • Physical health & development
    • Adaptive behavior/independent living
    • Nutrition
    • Vocational development
    • Social development
  • Acts as a role model for participants; furthers participants in researching and obtaining community resources appropriate and applicable to their needs and goals; follows-up on any aftercare/recommendations for treatment. Escorts/ accompanies participant as needed on appointments (medical, psychiatric, substance abuse).
  • Provide crisis intervention and mental health counseling as needed.
  • Participate in client meetings and conduct all 30, 60, 90 day follow-ups on discharged clients.
  • Required to participate and attend meetings/trainings at various sites that require following different route of travel to worksite.
  • Demonstrate ability to: maintain required written documentations; communicate relevant information to other staff and supervisor(s), verbally and in writing.
  • Ability to work flexible schedules as work schedules, trainings and location are subject to change
  • Ability to operate an agency vehicle.
  • Attend all required trainings and any relevant to the position or deemed necessary by supervisor.
  • Participate in activities of other staff members in their absence or during periods of staff shortage.
  • Maintain confidentiality at all times.
  • This does not limit the assignment of duties or exclude the performance of other duties not mentioned as assigned by supervisor, Deputy Director, Director, and/or Senior Management.
  • Responsible for reporting and documenting all incidents to immediate supervisor within 24 hours.
  • Qualifications
    • Master degree or higher in social work, psychology, rehabilitation counseling, or a related field, with licensure/registration. LMSW, LCSW, LMHC, RN (RNs are encouraged to be masters level or nurse practitioner), or physician (Ph.D, Psy. D, M.D.). Preference will be given to persons with experience on a multidisciplinary mobile team.
    • Two years supervisory experience with all levels of staff. Experienced in meeting regulatory requirements with OMH and other governmental agencies and working with behavioral health population preferred.
    • Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
    • Strong interpersonal and communication skills and knowledge of group dynamics required
    • Valid New York State drivers license and vehicle required candidates are subject to regulatory background checks.
