**JOB VACANCY NOTICE**
The mission of The CARE Center, Inc., is to provide a unique continuum of quality, recovery based, trauma informed, integrated services including education intervention, case management, treatment, aftercare and referrals to individuals and families in Butler, Greene, and Washington Counties.
**POSITION**: **Team Leader-Assertive Community Treatment** **Full Time**
The Assertive Community Treatment (ACT) provides comprehensive treatment to individuals 18 years of age and older with serious and persistent mental illness. It provides individualized services including case management, therapy, nursing, and psychiatric care.
**A Team Leader oversees a group of employees, motivates them to do their job efficiently, communicates any issues to Leadership to reach goals effectively, and will be a member of the team for after hours on-call rotation. As a Team Leader, you will develop and implement individualized treatment plans for clients and strive to lessen the symptoms of mental illness each individual client experiences and to minimize recurrent episodes of illness. Additionally, you will enhance the clients ability to live independently within the community.**

**QUALIFICATIONS:**
* Masters degree in social work, Psychology, or related social services field with at least 2 years of clinical experience.
* Demonstrated ability to obtain vital information during a personal interview to make proper recommendations.
* Demonstrated ability to establish and maintain effective working relationships with internal staff, providers, and other social service agencies.
* Must successfully obtain Criminal History Clearance from the Pennsylvania State Police and Child Abuse Clearance from the Pennsylvania Department of Human Services and FBI Fingerprint Clearance.
* Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
* Ability to transport oneself in an insured vehicle, as program needs dictate
